A third of a century after the confirmation of this new entity, the author gives a current review of the clinical, general and aetio-pathogenic characteristics and course, based upon personal research and the published literature. Nothing is to be added to or subtracted from the original description but all remains to be discovered as far as aetio-pathogenesis is concerned.
