Significant post-operative hemorrhage is a rare complication of intracranial surgery. Hemorrhage remote from the site of neurosurgical intervention is rare, with only 37 cases reported in the literature. We present the case of a 62 year old woman who presented a right occipital hematoma following excision of a large left sphenoidal meningioma. The hematoma was treated conservatively without neurological sequellae. We present a review of the literature concerning this rare phenomenon, and discuss the question of mechanical distorsion of intracranial structures with focal ischemia followed by reperfusion as the underlying pathophysiology.
