OBJECTIVE
To Study the operative approach and the method of exposure and cutting open for every lobar and segmental bile ducts and solve the surgical treatment of the complex multiple stones complicated with bile ducts stenosis in the right liver.
METHODS
Liver specimens of 30 adults were dissected to explore the lobar and segmental bile ducts and vessels. The exposure and cutting open of right intrahepatic bile ducts were well from the diaphragmatic face of the right hepatic lobe. This method was used in 10 patients with the disease.
RESULTS
All the patients were succesfully treated. Follow-up for 3-36 months, showed that 80% patients had excelent results, and 20% good ones.
CONCLUSION
The exposure and cutting open of the intrahepatic bile ducts from the diaphragmatic face of the right liver may take stones and solve bile duct stenosis in the every lobes and segment of the right liver.
目的
探讨各叶、段肝内胆管的手术入路及显露和切开方法，解决右肝复杂多发结石并胆管狭窄的外科治疗问题。
方法
解剖30例成人肝脏标本，探查肝叶、段胆管及血管。从右肝膈面良好显露并切开右肝内胆管。将此方法应用于10例患有该疾病的患者。
结果
所有患者均获成功治疗。随访3至36个月，结果显示80%的患者效果极佳，20%的患者效果良好。
结论
从右肝膈面显露并切开肝内胆管，可取出右肝各叶、段的结石并解决胆管狭窄问题。
