A case report is presented, which describes a kidney-transplant recipient medicated simultaneously with both cyclosporin A and nifedipine and who developed an extreme form of gingival hyperplasia. Repeated dental-surgical treatments provided only temporary regression of the gingival overgrowth, however, after discontinuation of the calcium channel blocker there was a significant improvement of the hyperplasia that no longer required specific dental treatment. The prevalence and mechanisms of this side effect in solid-organ transplant recipients on cyclosporin therapy is discussed. Treatment consists in the discontinuation of potentiating factors like calcium channel blockers and possibly also a dose reduction of cyclosporin, which together may support or reduce the need of dental-surgical procedures.
