BACKGROUND/AIMS: Mannose-binding lectin (MBL), a liver-derived C-type serum lectin, activates the complement cascade through the lectin pathway. Since the complement system contributes to the host defense against infections and mediates inflammatory processes including atherosclerosis, and since chronic renal failure (CRF) patients are prone to the development of infectious complications and cardiovascular disease, we focused on serum MBL levels in CRF patients who were either uremic, or who were receiving hemodialysis treatment.
MBL levels were measured in the sera of subjects with CRF before they began dialysis treatment (pre-HD patients; n = 23) and in the sera of subjects who were receiving maintenance hemodialysis (HD patients; n = 178), by ELISA using polyclonal anti-rabbit IgG and a monoclonal antibody directed against MBL (3E7).
Mean levels (+/- SD) of serum MBL were significantly higher in pre-HD subjects (4.343 +/- 2.533 microg/ml, p < 0.05) and in HD subjects (8.897 +/- 4.920 microg/ml, p < 0.05), than in healthy controls (1.452 +/- 0.692 microg/ml). Levels were also significantly higher in HD subjects than in pre-HD subjects (p < 0.05).
Elevated serum MBL levels in patients with CRF might have significantly influence pathologic conditions such as alterations of the immune system and acceleration of atherogenesis.
背景/目的:甘露糖结合凝集素(MBL)是一种肝脏来源的C型血清凝集素,通过凝集素途径激活补体级联反应。由于补体系统有助于宿主抵御感染并介导包括动脉粥样硬化在内的炎症过程,且慢性肾衰竭(CRF)患者易发生感染并发症和心血管疾病,我们重点关注了尿毒症或接受血液透析治疗的CRF患者的血清MBL水平。
采用多克隆抗兔IgG和针对MBL的单克隆抗体(3E7),通过ELISA法检测CRF患者透析治疗前(HD前患者;n = 23)和维持性血液透析患者(HD患者;n = 178)血清中的MBL水平。
HD前患者血清MBL的平均水平(±标准差)(4.343±2.533μg/ml,p < 0.05)和HD患者(8.897±4.920μg/ml,p < 0.05)均显著高于健康对照(1.452±0.692μg/ml)。HD患者的水平也显著高于HD前患者(p < 0.05)。
CRF患者血清MBL水平升高可能对免疫系统改变和动脉粥样硬化加速等病理状况产生显著影响。
