To evaluate the efficacy and safety of low molecular weight heparin(LMWH) in prophylaxis of postoperative deep vein thrombosis (DVT) following hip and knee surgery.
From April 1997 to October 1998, 46 patients undergoing hip and knee orthopedic procedures were randomized into 2 groups for studying. The following eligibility criteria were applied: age over 40 years old, no recently history of venous thromboembolism (over 3 months), normal result of preoperative hemostasis test and normal result of Doppler examination of the lower extremities. One group was control group and the other group received subcutaneously a low molecular weight heparin(Fraxiparine) with anti-factor X, activity of 41 IU/kg.day for three days, then 62 IU/kg.day from the 4th day to 10th day. All patients had venegraphy performed in the operated leg at 4 to 7 days after surgery.
eight patients(34.8%) developed DVT in the control group of 23 patients and 1 patient (4.3%) in the experimental group, also of 23 patients(P < 0.05). Two groups had no any bleeding complications.
The low molecular weight heparin is safe and effective in preventing postoperative deep vein thrombosis in patients following hip and knee surgery.
评估低分子量肝素(LMWH)预防髋膝关节手术后深静脉血栓形成(DVT)的有效性和安全性。
1997年4月至1998年10月,46例行髋膝关节矫形手术的患者被随机分为两组进行研究。采用以下入选标准:年龄40岁以上,近期无静脉血栓栓塞病史(超过3个月),术前止血试验结果正常,下肢多普勒检查结果正常。一组为对照组,另一组皮下注射具有抗X因子活性的低分子量肝素(速碧林),剂量为41 IU/kg·天,共3天,从第4天至第10天为62 IU/kg·天。所有患者在术后4至7天对手术侧下肢进行静脉造影。
对照组23例患者中有8例(34.8%)发生DVT,试验组23例患者中有1例(4.3%)发生DVT(P<0.05)。两组均无出血并发症。
低分子量肝素预防髋膝关节手术后患者深静脉血栓形成安全有效。
