Although both interleukin-4 (IL-4) and IL-13 induce immunoglobulin E (IgE) production in vitro, the relative contribution of the two cytokines in various skin lesions of atopic dermatitis remains unclear.
We examined the relative importance of IL-4 and IL-13 in lesional skin of atopic dermatitis for IgE production.
The study group comprised 28 atopic dermatitis patients with serum IgE levels more than 2000 IU/ml. The expression levels of IL-4 mRNA and IL-13 mRNA versus that of glyceraldehyde-3-phosphate dehydrogenase (GAPDH) were determined using a semiquantitative reverse transcription-polymerase chain reaction (RT-PCR) method in samples of normal-appearing skin from seven patients, very mild lesions from eight patients, subacute lesions from ten patients and lichenified lesions from ten patients with atopic dermatitis, and in samples of normal skin from six healthy control subjects.
IL-4 mRNA expression was identified in only two of the eight very mild lesions, one of the ten subacute lesions, and none of the ten lichenified lesions, whereas IL-13 mRNA expression was identified in 27 of the 28 skin lesions of atopic dermatitis. The IL-13 mRNA/GAPDH mRNA ratios (x100) in the subacute lesions (94.4+/-20.6) and lichenified lesions (71.4+/-40.4) were significantly greater than in the skin of healthy controls (13.1+/-17.7; P<0.01, P<0.05, respectively).
Upregulation of IL-13 mRNA in subacute and chronic lesions of atopic dermatitis along with scant expression of IL-4 mRNA suggest that IL-13 is a crucial cytokine in lesional skin.

我们研究了IL-4和IL-13在特应性皮炎皮损中对IgE产生的相对重要性。
研究组包括28例血清IgE水平超过2000 IU/ml的特应性皮炎患者。采用半定量逆转录-聚合酶链反应(RT-PCR)法,测定了7例特应性皮炎患者外观正常皮肤、8例极轻度皮损、10例亚急性皮损和10例苔藓化皮损样本中IL-4 mRNA和IL-13 mRNA相对于甘油醛-3-磷酸脱氢酶(GAPDH)的表达水平,并测定了6例健康对照者正常皮肤样本中的表达水平。
在8例极轻度皮损中仅2例检测到IL-4 mRNA表达,10例亚急性皮损中1例检测到,10例苔藓化皮损中均未检测到;而在28例特应性皮炎皮肤损害中有27例检测到IL-13 mRNA表达。亚急性皮损(94.4±20.6)和苔藓化皮损(71.4±40.4)中IL-13 mRNA/GAPDH mRNA比值(×100)显著高于健康对照者皮肤(13.1±17.7;P均<0.01)。
特应性皮炎亚急性和慢性皮损中IL-13 mRNA上调而IL-4 mRNA表达稀少,提示IL-13是皮损中的关键细胞因子。
