A patient with tetralogy of Fallot was found to have pulmonary sling and an anomalous connection of the left subclavian artery with the pulmonary artery. At birth, she had a left split-hand deformity with preaxial (radial) syndactyly. Anomalous connection of a systemic vessel with the pulmonary artery is an uncommon cardiovascular defect. It is suggested that this cardiac defect may have caused intrauterine limb ischemia and the hand defect.
