Osteopontin (OPN) is a secreted integrin-binding glycophosphoprotein that may have a role in head and neck squamous cell carcinoma (SCC). To evaluate the clinical significance of OPN in esophageal squamous cell carcinoma (ESCC), we compared plasma OPN levels with those of common tumor markers.
Preoperative plasma OPN levels were measured by enzyme immunoassay in 103 ESCC patients. Serum SCC antigen, Cyfra 21-1, and carcinoembryonic antigen (CEA) levels were also measured routinely at admission by radioimmunoassay.
Plasma OPN levels ranged from 82.8 to 1,980 ng/ml. High OPN level was associated with lymph node metastasis (p = 0.05), but not with tumor histology or depth of invasion. The overall survival of the patients with high OPN levels was worse than that of those with low OPN levels (p = 0.02). SCC antigen and Cyfra 21-1 levels were associated with the depth of tumor invasion, the tumor diameter, lymph node metastasis, and the overall survival, but CEA was not associated with these clinicopathological factors. Combined evaluation of OPN plus Cyfra 21-1 or OPN plus SCC antigen was useful as an independent prognostic indicator.
Measurement of the plasma OPN level, as well as serum SCC antigen and Cyfra 21-1, may help to predict the progression of ESCC.
骨桥蛋白（OPN）是一种分泌型整合素结合糖磷蛋白，可能在头颈部鳞状细胞癌（SCC）中发挥作用。为评估OPN在食管鳞状细胞癌（ESCC）中的临床意义，我们将血浆OPN水平与常见肿瘤标志物的水平进行了比较。
采用酶免疫分析法测定了103例ESCC患者术前血浆OPN水平。入院时还常规采用放射免疫分析法测定血清鳞状细胞癌抗原、细胞角蛋白19片段（Cyfra 21-1）和癌胚抗原（CEA）水平。
血浆OPN水平范围为82.8至1980 ng/ml。OPN水平高与淋巴结转移相关（p = 0.05），但与肿瘤组织学或浸润深度无关。OPN水平高的患者总生存期比OPN水平低的患者差（p = 0.02）。鳞状细胞癌抗原和Cyfra 21-1水平与肿瘤浸润深度、肿瘤直径、淋巴结转移及总生存期相关，但CEA与这些临床病理因素无关。联合评估OPN加Cyfra 21-1或OPN加鳞状细胞癌抗原可作为独立的预后指标。
检测血浆OPN水平以及血清鳞状细胞癌抗原和Cyfra 21-1水平可能有助于预测ESCC进展。
