To report the unexpected finding of a primary retinal B-cell lymphoma in a patient with previous breast carcinoma and subsequent lymphoma of the stomach.
Histopathologic and immunohistochemical findings in the enucleated eye are described in conjunction with clinical features and investigations.
An 80-year-old woman with previous breast cancer presented with moderate loss of vision associated with vitreous opacities, circumpapillary retinal haemorrhages, and attenuated retinal vessels in her left eye. Treatment with systemic steroids and oral immunosuppressive drugs was initiated on suspicion of carcinoma-associated retinopathy. Two years later, the patient was diagnosed with MALT lymphoma of the stomach. Unfortunately, the ocular symptoms did not improve, and the patient developed neovascular glaucoma, for which the eye was eventually enucleated. Histopathology and immunohistochemistry as well as a molecular analysis revealed a primary B-cell lymphoma of the retina.
Non-Hodgkin B-cell lymphoma of the retina should be considered as a differential diagnosis in cases of non-specific retinopathies even in the presence of other primary neoplastic co-morbidities.

一名80岁既往有乳腺癌的女性，左眼出现与玻璃体混浊、视乳头周围视网膜出血及视网膜血管变细相关的中度视力丧失。因怀疑癌相关性视网膜病变，开始给予全身类固醇和口服免疫抑制药物治疗。两年后，患者被诊断为胃黏膜相关淋巴组织淋巴瘤。不幸的是，眼部症状未改善，患者发展为新生血管性青光眼，最终眼球被摘除。组织病理学、免疫组化及分子分析显示为原发性视网膜B细胞淋巴瘤。
即使存在其他原发性肿瘤合并症，在非特异性视网膜病变病例中，视网膜非霍奇金B细胞淋巴瘤也应被视为鉴别诊断之一。
