A 60-year-old man with senile cataract had phacoemulsification and implantation of a single-piece acrylic intraocular lens (IOL) in July 2000. There were no intraoperative or short-term postoperative complications, and the visual acuity was 20/20 on the day after surgery. In February 2002, 19 months postoperatively, posterior capsule opacification (PCO) extending from the base of the IOL loops was observed. Capsulotomy was not performed, and the PCO regressed spontaneously between April 2003 and March 2005. This is the first documented case of spontaneous PCO regression after phacoemulsification and IOL implantation.
