Data indicate that patient outcomes are significantly affected by factors that can be modified through clinical practices, such as vascular access choice, dialysis dose, interdialytic weight gain, albumin and hemoglobin levels, and mineral and bone disorders. This article reviews recommended treatment guidelines for these factors and the ways patient outcomes are affected when targets are not achieved. Insights on how nursing practices can positively affect these components of care are also provided.
