Our healthcare service system which used to have a strictly sectoral structure experiences a radical change. Office-based specialists cannot any longer rely on the fact that they have exclusive responsibility for the ambulatory treatment of patients. In the past years the legislator has melted the strict separation between hospital treatment and ambulatory care. Finally the establishment of medical service centres (MVZ) and the opening of hospitals for highly specialized services and the treatment of special diseases relating to the Fifth Book of Social Code, Para. 116 b, (SGB V) have dramatically changed the position of office-based specialists. The legislative measures aim at eliminating the so-called "double specialist track2 (doppelte Facharztschiene)". So far no satisfying solution to this new conflict has been found.
我们过去有着严格部门结构的医疗服务体系正在经历根本性变革。以诊所为基础的专科医生再也不能依赖他们对患者门诊治疗负有独家责任这一事实。在过去几年里,立法者打破了医院治疗和门诊护理之间的严格界限。最终,医疗服务中心(MVZ)的建立以及开设针对高度专业化服务和与《社会法典》第五卷第116 b条(SGB V)相关特殊疾病治疗的医院,极大地改变了以诊所为基础的专科医生的地位。这些立法措施旨在消除所谓的“双重专科路径(doppelte Facharztschiene)”。到目前为止,尚未找到解决这一新冲突的令人满意的办法。
