Abstract

PURPOSE

To evaluate the therapeutic impact of [(18)F]fluoride positron-emission tomography/computed tomography ([(18)F]fluoride PET/CT) imaging on patients with unclear foot pain.

METHODS

Twenty-eight patients were prospectively included in this study. Therapeutic management was defined by two experienced dedicated foot surgeons before and after [(18)F]fluoride PET/CT imaging. Twenty-six patients underwent cross-sectional imaging [CT, magnetic resonance (MR)] prior to PET/CT. A retrospective analysis of the magnetic resonance imaging (MRI) diagnoses was performed when a therapy change occurred after PET/CT imaging.

RESULTS

In 13/28 (46%) patients therapeutic management was changed due to PET/CT results. Management changes occurred in patients with the following diagnoses: os trigonum syndrome; sinus tarsi syndrome; os tibiale externum syndrome; osteoarthritis of several joints; non-consolidated fragments; calcaneo-navicular coalition; plantar fasciitis; insertional tendinopathy; suggestion of periostitis; neoarticulations between metatarsal bones. Os trigonum, os tibiale externum, subtalar osteoarthritis and plantar fasciitis were only seen to be active on PET/CT images but not on MR images.

CONCLUSION

[(18)F]fluoride PET/CT has a substantial therapeutic impact on management in patients with unclear foot pain.

目的

评估氟[(18)F]正电子发射断层扫描/计算机断层扫描(氟[(18)F] PET/CT)成像对不明原因足部疼痛患者的治疗影响。

方法

本研究前瞻性纳入 28 例患者。在氟[(18)F] PET/CT 成像前后,由两位经验丰富的专门足部外科医生定义治疗管理。26 例患者在 PET/CT 前进行了横断面成像(CT、磁共振(MR))。当 PET/CT 成像后治疗发生变化时,对 MRI 诊断进行回顾性分析。

结果

在 28 例患者中的 13 例(46%)中,由于 PET/CT 结果改变了治疗管理。管理变化发生在以下诊断的患者中:距骨后三角骨综合征;跗骨窦综合征;外踝骨综合征;多个关节骨关节炎;未愈合碎片;跟舟骨联合;足底筋膜炎;插入性肌腱病;骨膜炎提示;跖骨间新关节。只有在氟[(18)F] PET/CT 图像上而不是在 MR 图像上观察到距骨后三角骨、外踝骨、距下关节炎和足底筋膜炎活跃。

结论

氟[(18)F] PET/CT 对不明原因足部疼痛患者的管理具有重要的治疗影响。
