There is evidence that anal sex is becoming increasingly popular among heterosexual women and men. Several studies carried out in especially vulnerable populations (e.g. sex workers and low-income youth) suggested that anal sex may indicate a more general propensity to sexual risk-taking.
To assess whether this epidemiologically important finding holds in the case of young adults from the general population, we analysed data from a cross-sectional probability survey carried out in 2010 on 1005 Croatian women and men aged 18-25.
Anal intercourse was reported by 36.5% of 861 sexually experienced participants (42.7% of men and 29.8% of women). About one-third of them (34%) used a condom at most recent anal intercourse. The experience of anal sex was significantly associated (P<0.001) with all four indicators of sexual risk-taking (condom use at most recent vaginal intercourse, number of sexual partners in the past year, concurrent sexual relationships and anonymous sex in the past year), as well as with negative attitudes and beliefs about condom use (P<0.01). Sexual sensation-seeking mediated the relationship between anal sex and some of the sexual risk-taking behaviours.
According to the findings, heterosexual anal sex is directly and indirectly associated with increased behavioural risks of acquiring HIV and other sexually transmissible infections (STI). Sex education and STI prevention programs should focus on the importance of using protection when practicing anal sex.

为评估这一在流行病学上具有重要意义的发现是否适用于普通人群中的年轻人，我们分析了2010年对1005名年龄在18至25岁的克罗地亚男女进行的一项横断面概率调查的数据。
在861名有性经历的参与者中，36.5%的人报告有过肛交（男性为42.7%，女性为29.8%）。其中约三分之一（34%）的人在最近一次肛交时使用了避孕套。肛交经历与所有四项性冒险指标（最近一次阴道性交时使用避孕套、过去一年中的性伴侣数量、同时存在的性关系以及过去一年中的匿名性行为）均显著相关（P<0.001），也与对使用避孕套的负面态度和观念显著相关（P<0.01）。寻求性刺激在肛交与某些性冒险行为之间起到了中介作用。
根据研究结果，异性肛交与感染艾滋病毒和其他性传播感染（STI）的行为风险增加直接和间接相关。性教育和性传播感染预防项目应关注在进行肛交时使用保护措施的重要性。
