Abstract

OBJECTIVE

To examine using the folds of the retinal pigment epithelium (RPE) to diagnose Vogt-Koyanagi-Harada (VKH) disease.

METHODS

Retrospective clinical review of 57 Japanese patients between July 2005 and April 2009. Optical coherence tomography (OCT), fundus photographs and fluorescein angiography (FA) were studied to investigate Kendall's coefficients of concordance in seven findings by four ophthalmologists. The sensitivity and specificity of each finding were examined to confirm its efficacy. Each result was defined as positive when more than three of the ophthalmologists judged that the finding existed.

RESULTS

The folds of the RPE on OCT were observed in 30/42 (71.4 %) eyes having VKH disease, but none in the other 72 eyes. Kendall's coefficient of concordance in the detection of the folds of the RPE was 0.90 and was the highest among all other findings. Sensitivity was higher in the detection of the RPE folds on OCT and in optic nerve staining on FA than other findings and the specificity was 100 % for the RPE folds and 26.7 % for the optic nerve staining.

CONCLUSION

The detection of the folds of the RPE on OCT is a simple and effective method to help diagnose VKH disease at its acute stage which does not require pupil dilation.

目的

利用视网膜色素上皮(RPE)的皱褶来诊断 Vogt-小柳原田(VKH)病。

方法

回顾性分析 2005 年 7 月至 2009 年 4 月间的 57 例日本患者。通过四位眼科医生研究光学相干断层扫描(OCT)、眼底照片和荧光素血管造影(FA),以研究七种发现的 Kendall 一致性系数。检查每种发现的敏感性和特异性以确认其效果。当超过三位眼科医生判断存在该发现时,每个结果均定义为阳性。

结果

在 30/42(71.4%)只患有 VKH 病的眼中观察到 OCT 上的 RPE 皱褶,但在另外 72 只眼中没有观察到。OCT 上 RPE 皱褶的检测 Kendall 一致性系数为 0.90,是所有其他发现中最高的。OCT 上 RPE 皱褶的检测敏感性高于 FA 上视神经染色的检测敏感性,而 RPE 皱褶的特异性为 100%,视神经染色的特异性为 26.7%。

结论

OCT 上 RPE 皱褶的检测是一种简单有效的方法,可以帮助在不需要散瞳的情况下诊断 VKH 病的急性期。
