Endolaryngeal synovial sarcoma has only been reported in the literature three times. We add to this a fourth report of a 28-year-old woman. This case distinguishes itself by being a monophasic variant and by being the first to be substantiated with extensive immunohistochemical analysis. Treatment consisted of shelling-out followed by radiotherapy, which is a variant from standard therapy. The patient is tumor-free 3 years after therapy.
