Dyssynergic defecation is a major cause of chronic functional constipation as a common digestive complaint. We clinically evaluated the effects of biofeedback therapy on the pelvic floor motion indices through magnetic resonance (MR) defecography, quality of life and depression in patients with dyssynergic defecation.
In this clinical trial study, among patients referring to the Colorectal Clinic of Hazrat Rasoul Hospital, 22 subjects were randomly assigned into two equal groups (n= 11) to receive either standard only or biofeedback and standard therapy. Dynamic changes of the pelvic floor were measured by MR defecography. During the simulated defecation, two MR defecography dynamic indices including abnormal anorectal angle change and perineal descent were measured before and after treatment. The effects of biofeedback therapy on patients' symptoms, quality of life and severity of depression were assessed and compared with the standard therapy. Statistical analysis was carried out using independent _t-test and Mann-Whitney test.
Paradox index (p< 0.001), perineal descent index (p< 0.001), depression (p< 0.1), physical function (p< 0.001), vitality (p< 0.001) and role emotion (p< 0.001) significantly improved in the biofeedback therapy group in contrast to the standard therapy SDT group.
Biofeedback therapy appears to be effective in improving symptoms of functional constipation and dysfunction of pelvic floor motion as well as patient's quality of life and depression state. MR defecography is able to show the changes in dynamic indices of the pelvic floor through biofeedback therapy.
排便协同失调是慢性功能性便秘这一常见消化问题的主要原因。我们通过磁共振(MR)排粪造影、生活质量和抑郁情况,对排便协同失调患者盆底运动指标的生物反馈疗法效果进行了临床评估。
在这项临床试验研究中,在转诊至拉苏勒医院结直肠诊所的患者中,22名受试者被随机分为两组(n = 11),分别接受单纯标准治疗或生物反馈加标准治疗。通过MR排粪造影测量盆底的动态变化。在模拟排便过程中,测量治疗前后两个MR排粪造影动态指标,包括异常肛管直肠角变化和会阴下降。评估生物反馈疗法对患者症状、生活质量和抑郁严重程度的影响,并与标准疗法进行比较。使用独立t检验和曼-惠特尼检验进行统计分析。
与标准治疗SDT组相比,生物反馈治疗组的矛盾指数(p < 0.001)、会阴下降指数(p < 0.001)、抑郁(p < 0.1)、身体功能(p < 0.001)、活力(p < 0.001)和角色情绪(p < 0.001)显著改善。
生物反馈疗法似乎对改善功能性便秘症状、盆底运动功能障碍以及患者生活质量和抑郁状态有效。MR排粪造影能够通过生物反馈疗法显示盆底动态指标的变化。
