Abstract

Endonasal endoscopic sinus surgery is the standard procedure for surgery of most paranasal sinus diseases. Appropriate frame conditions provided, the respective procedures are safe and successful. These prerequisites encompass appropriate technical equipment, anatomical oriented surgical technique, proper patient selection, and individually adapted extent of surgery. The range of endonasal sinus operations has dramatically increased during the last 20 years and reaches from partial uncinectomy to pansinus surgery with extended surgery of the frontal (Draf type III), maxillary (grade 3-4, medial maxillectomy, prelacrimal approach) and sphenoid sinus. In addition there are operations outside and beyond the paranasal sinuses. The development of surgical technique is still constantly evolving. This article gives a comprehensive review on the most recent state of the art in endoscopic sinus surgery according to the literature with the following aspects: principles and fundamentals, surgical techniques, indications, outcome, postoperative care, nasal packing and stents, technical equipment.

摘要

鼻内镜鼻窦手术是大多数鼻窦疾病的标准手术方式。在具备适当条件的情况下,相应的手术是安全且成功的。这些前提条件包括合适的技术设备、以解剖为导向的手术技术、恰当的患者选择以及个体化调整的手术范围。在过去20年中,鼻内镜鼻窦手术的范围显著扩大,从部分钩突切除术到全鼻窦手术,包括扩大的额窦手术(Draf III型)、上颌窦手术(3 - 4级、内侧上颌骨切除术、泪前入路)和蝶窦手术。此外,还有鼻窦外及鼻窦以外区域的手术。手术技术仍在不断发展。本文根据文献,从以下几个方面对内窥镜鼻窦手术的最新技术水平进行全面综述:原则与基础、手术技术、适应证、疗效、术后护理、鼻腔填塞与支架、技术设备。
