To clarify the functional status of the articularis genus muscle (AGM) in individuals with knee osteoarthritis (OA) and to analyze the muscle's relationship with knee OA.
Fifty-two individuals with knee OA (mean age, 73.4 years), 50 elderly individuals without knee OA changes (mean age, 71.2 years) and 75 young individuals (mean age, 20.2 years) were observed the AGM using ultrasonography. The thickness of the AGM, the anteroposterior distance of the suprapatellar bursa, and moving distance of the muscle insertion were measured both at rest and during isometric contraction, and values during contraction were expressed as percentages of the values at rest (%Muscle-Increase, %Bursa-Increase).
Muscle thickness at rest, %Muscle-Increase, %Bursa-Increase, and moving distance of the muscle insertion were significantly lower and anteroposterior distance of the suprapatellar bursa was significantly higher in the OA group than in the controls (p<0.001, all). In the OA group, these values for the AGM were significantly correlated with knee range of motion, knee pain, and Kellgren and Lawrence grade.
Individuals with knee OA exhibited atrophic changes and dysfunctions of the AGM, and these were associated with symptoms. Atrophic changes and dysfunctions of the AGM may be specific changes associated with knee OA.
阐明膝关节骨关节炎(OA)患者股关节肌(AGM)的功能状态,并分析该肌肉与膝关节OA的关系。
采用超声检查观察52例膝关节OA患者(平均年龄73.4岁)、50例无膝关节OA改变的老年人(平均年龄71.2岁)和75例年轻人(平均年龄20.2岁)的AGM。在静息状态和等长收缩期间测量AGM的厚度、髌上囊的前后径以及肌肉附着点的移动距离,收缩期间的值表示为静息值的百分比(%肌肉增加、%滑囊增加)。
OA组静息时的肌肉厚度、%肌肉增加、%滑囊增加和肌肉附着点的移动距离均显著低于对照组,髌上囊的前后径显著高于对照组(均p<0.001)。在OA组中,AGM的这些值与膝关节活动范围、膝关节疼痛以及Kellgren和Lawrence分级显著相关。
膝关节OA患者表现出AGM的萎缩性改变和功能障碍,且这些与症状相关。AGM的萎缩性改变和功能障碍可能是与膝关节OA相关的特异性改变。
