Abstract

To investigate the association of occupational stress and 5-hydroxytryptamine 2A (5-HT2A) receptor gene polymorphisms with depression. In November 2010, cluster sampling was used to select 589 workers in a thermal power plant as study subjects. Questionnaires were used to investigate demographic features and occupational stressors. Polymerase chain reaction-restriction fragment length polymorphism was used to detect the genotypes of T102C and A-1438G in 5-HT2A receptor gene in 589 workers. High-level daily hassles (=3.013, 95% 1.6425.530) , more negative emotion (=4.808, 95% 2.6628.681) , more body needs (=1.890, 95% 1.0343.453) , and severe role conflict (=1.815, 95% 1.0023.288) were risk factors for depression, while high rewards (=0.424, 95% 0.2260.796) was the protective factor against depression (all <0.05). There were no significant differences in T102C genotype and allele distributions between the groups with and without depression (>0.05) ; there was a significant difference in A-1438G genotype distribution between the groups with and without depression (χ= 9.573, <0.05) , while there was no significant difference in A-1438G allele distribution between these groups (>0.05). The risk of depression in the workers with high-level daily hassles who carried TC genotype (= 4.473, 95% 1.16117.238) or CC genotype (=5.176, 95% 1.36719.593) of T102C was 4.473 and 5.176 times that in those with low-level daily hassles who carried TT genotype, and the risk of depression in the workers with more negative emotions who carried TC genotype (=5.667, 95% 1.20426.673) or CC genotype (=8.114, 95% 1.74737.677) of T102C was 5.667 and 8.114 times that in those with less negative emotion who carried TT genotype. The risk of depression in the workers with high-level daily hassles who carried AG genotype (=4.505, 95% 2.2159.162) or GG genotype (=6.484, 95% 2.562~ 16.414) of A-1438G was 4.505 and 6.484 times that in those with low-level daily hassles who carried AA genotype, and the risk of depression in the workers with more negative emotion who carried AG genotype (= 4.877, 95% 2.32610.223) or GG genotype (=9.090, 95% 3.49123.673) of A-1438G was 4.877 and 9.090 times that in those with less negative emotion who carried AA genotype. The risk of depression in the workers with higher rewards who carried AG genotype (=0.152, 95% 0.0740.310) or GG genotype (= 0.384, 95% 0.1530.964) of A-1438G was 0.152 and 0.384 times that in those with lower rewards who carried AA genotype. Occupational stress is associated with the development of depression and interacts with 5-HT2A receptor gene polymorphisms.

探讨职业应激和5-羟色胺2A(5-HT2A)受体基因多态性与抑郁症的关联。2010年11月,采用整群抽样法选取某火力发电厂589名工人作为研究对象。采用问卷调查法调查人口学特征和职业应激源。采用聚合酶链反应-限制性片段长度多态性方法检测589名工人5-HT2A受体基因T102C和A-1438G的基因型。高水平日常困扰(=3.013,95%可信区间1.6425.530)、更多负面情绪(=4.808,95%可信区间2.6628.681)、更多身体需求(=1.890,95%可信区间1.0343.453)和严重角色冲突(=1.815,95%可信区间1.0023.288)是抑郁症的危险因素,而高回报(=0.424,95%可信区间0.2260.796)是抑郁症的保护因素(均P<0.05)。抑郁组与非抑郁组T102C基因型和等位基因分布差异无统计学意义(P>0.05);抑郁组与非抑郁组A-1438G基因型分布差异有统计学意义(χ=9.573,P<0.05),而两组间A-1438G等位基因分布差异无统计学意义(P>0.05)。携带T102C的TC基因型(=4.473,95%可信区间1.16117.238)或CC基因型(=5.176,95%可信区间1.36719.593)且日常困扰水平高的工人患抑郁症的风险是携带TT基因型且日常困扰水平低的工人的4.473倍和5.176倍,携带T102C的TC基因型(=5.667,95%可信区间1.20426.673)或CC基因型(=8.114,95%可信区间1.74737.677)且负面情绪较多的工人患抑郁症的风险是携带TT基因型且负面情绪较少的工人的5.667倍和8.114倍。携带A-1438G的AG基因型(=4.505,95%可信区间2.2159.162)或GG基因型(=6.484,95%可信区间2.56216.414)且日常困扰水平高的工人患抑郁症的风险是携带AA基因型且日常困扰水平低的工人的4.505倍和6.484倍,携带A-1438G的AG基因型(=4.877,95%可信区间2.32610.223)或GG基因型(=9.090,95%可信区间3.49123.673)且负面情绪较多的工人患抑郁症的风险是携带AA基因型且负面情绪较少的工人的4.877倍和9.090倍。携带A-1438G的AG基因型(=0.152,95%可信区间0.0740.310)或GG基因型(=0.384,95%可信区间0.153~0.964)且回报较高的工人患抑郁症的风险是携带AA基因型且回报较低的工人的0.152倍和0.384倍。职业应激与抑郁症的发生有关,并与5-HT2A受体基因多态性相互作用。
