We describe a very rare case of late-stage parenchymatous neurosyphilis, presenting as general paresis of insane, diagnosed in a 44-year-old male truck driver. Probable place of exposition was Belarus. An appropriate clinical examination, laboratory diagnostics, and subsequent effective treatment and follow-up in accordance with evidence-based management guidelines are essential to avoid irreversible sequelae in these late-stage neurosyphilis patients. Increased awareness that rare cases of parenchymatous neurosyphilis still exist in the 21st century is crucial and this entity should still be considered in differential diagnosis in patients with neuropsychiatric symptoms such as memory or cognitive impairment.
我们描述了一例非常罕见的晚期实质性神经梅毒病例,表现为麻痹性痴呆,患者为一名44岁的男性卡车司机,可能的感染地是白俄罗斯。进行适当的临床检查、实验室诊断,并根据循证管理指南进行后续有效的治疗和随访,对于避免这些晚期神经梅毒患者出现不可逆的后遗症至关重要。认识到21世纪仍存在罕见的实质性神经梅毒病例至关重要,对于有记忆或认知障碍等神经精神症状的患者进行鉴别诊断时仍应考虑这一疾病。
