Abstract

BACKGROUND

Multiple treatment options are introduced in treatment of vitiligo but the response is unsatisfactory.

OBJECTIVE

In this prospective, randomized, comparative trial, we studied the effect of combined treatment with fractional carbon dioxide (CO ) laser, platelet-rich plasma (PRP) injection, and narrowband ultraviolet B (NB-UVB) for stable nonsegmental vitiligo regarding repigmentation grade, patient's satisfaction, and side effects.

METHODS

Eighty adult patients with localized nonsegmental vitiligo were enrolled in this study. The patients were randomly categorized to receive 4 lines of treatment; fractional CO laser, PRP, combined fractional CO laser and PRP, and combined fractional CO laser and NB-UVB. The treatment period was 2 months. Patients were clinically evaluated 3 months after the last treatment. Outcome was evaluated by 5-point scale for repigmentation, 10-point visual analog scale for patient's satisfaction, and side effects.

RESULTS

Laser and PRP group achieved the best results regarding repigmentation and patient's satisfaction. Sixty percent of the patients developed repigmentation >50% and 40% of patients developed repigmentation >75%. In laser and NB-UVB group, 5% developed repigmentation >75% and 25% developed repigmentation >50%. Only 10% of patients developed repigmentation >75% in laser group and only 20% of patients developed repigmentation >75% in PRP group.

CONCLUSIONS

Combination of fractional CO laser with PRP injection is a promising treatment for vitiligo, followed by combination of fractional CO laser with NB-UVB phototherapy. Both fractional CO laser and PRP injection gave poor results if they received alone.

背景

白癜风的治疗有多种选择,但疗效不尽人意。

目的

在这项前瞻性、随机、对照试验中,我们研究了分次二氧化碳(CO₂)激光、富血小板血浆(PRP)注射和窄谱中波紫外线(NB-UVB)联合治疗稳定期非节段性白癜风的色素恢复程度、患者满意度及副作用。

方法

80例成年局限性非节段性白癜风患者纳入本研究。患者被随机分为4组接受治疗:分次CO₂激光、PRP、分次CO₂激光联合PRP、分次CO₂激光联合NB-UVB。治疗期为2个月。最后一次治疗3个月后对患者进行临床评估。通过色素恢复的5分制量表、患者满意度的10分视觉模拟量表及副作用来评估结果。

结果

激光联合PRP组在色素恢复和患者满意度方面取得了最佳效果。60%的患者色素恢复>50%,40%的患者色素恢复>75%。在激光联合NB-UVB组,5%的患者色素恢复>75%,25%的患者色素恢复>50%。激光组仅有10%的患者色素恢复>75%,PRP组仅有20%的患者色素恢复>75%。

结论

分次CO₂激光联合PRP注射是一种有前景的白癜风治疗方法,其次是分次CO₂激光联合NB-UVB光疗。单独使用分次CO₂激光或PRP注射效果均不佳。
