The purposes of this study were to compare clinical results after total knee arthroplasty (TKA) using the Attune and PFC Sigma knee designs and to investigate whether the use of the Attune prosthesis increased the risk of patellar injury in Asian patients.
Three hundred knees that underwent TKA using Attune (group A) were compared to 300 knees that underwent TKA using PFC Sigma (group B). The Knee Society Knee Score (KS) and Function Score (FS), and range of motion (ROM) were compared. The residual patellar thickness was compared to evaluate the risk of patellar injury.
The postoperative KS and ROM of group A were better than those of group B (93.1 vs. 88.8, p<0.001 and 131.4° vs. 129.0°, p=0.008, respectively). The postoperative FS did not differ significantly between the two groups (80.9 vs. 78.7, p=0.427). The residual patella was thinner in group A (14.8 mm vs. 15.7 mm, p=0.003), which made up a higher proportion of the high-risk group for patellar fractures with a residual thickness of <12 mm (7.5% vs. 2.1%, p=0.003).
TKA using the Attune prosthesis provided more favorable clinical results than TKA using PFC Sigma. However, the risk of injury in the residual patella was increased with use of the Attune prosthesis in Asian patients.
本研究的目的是比较使用Attune和PFC Sigma膝关节设计进行全膝关节置换术(TKA)后的临床结果,并调查使用Attune假体是否会增加亚洲患者髌骨关节损伤的风险。
将300例使用Attune进行TKA的膝关节(A组)与300例使用PFC Sigma进行TKA的膝关节(B组)进行比较。比较膝关节协会膝关节评分(KS)、功能评分(FS)和活动范围(ROM)。比较残余髌骨厚度以评估髌骨关节损伤的风险。
A组术后KS和ROM均优于B组(分别为93.1对88.8,p<0.001;131.4°对129.0°,p = 0.008)。两组术后FS差异无统计学意义(80.9对78.7,p = 0.427)。A组残余髌骨更薄(14.8 mm对15.7 mm,p = 0.003),在残余厚度<12 mm的髌骨骨折高危组中占比更高(7.5%对2.1%,p = 0.003)。
使用Attune假体的TKA比使用PFC Sigma的TKA提供了更有利的临床结果。然而,在亚洲患者中使用Attune假体增加了残余髌骨关节损伤的风险。
