The effects of nanocurcumin on Treg cell responses and treatment of ankylosing spondylitis patients: A randomized, double-blind, placebo-controlled clinical trial.

Abstract

AIM

Ankylosing spondylitis (AS) is an inflammatory rheumatic disease with increased bone mass in the main sites of inflammation. Regulatory T (Treg) cells have been reported to involve in pathology of AS. This study designed at investigating the effects of nanocurcumin on Treg cell responses in peripheral blood (PB) of AS patients.

METHODS

Test group including 12 AS patients received nanocurcumin daily for 4 months and control group including 12 patients received placebo. The frequency of Treg was measured by flow cytometry. The expression levels of FoxP3 and several associated microRNAs (miRNAs; miR-27, miR-17, and miR-146a) and cytokines including Interleukin-10 (IL-10), TGF-β, and IL-6 were assessed by real-time polymerase chain reaction. Furthermore, enzyme-linked immunosorbent assay was done to determine the secretion levels of cytokines.

RESULTS

After treatment with nanocurcumin the frequency of Treg cells in AS patients increased significantly. The RT-PCR data indicated that the expression of miR-17 and miR-27 were significantly decreased following nanocurcumin treatment while miR-146a and FoxP3 were significantly increased. Moreover, nanocurcumin-treated group had high levels of IL-10 and TGF-β and low levels of IL-6 production than control group.

CONCLUSION

The findings suggested that dysregulation of Treg cells in PB influences the AS development and nanocurcumin therapy could regulate the Treg cells, and so could be useful in the treatment of AS and may be other autoimmune diseases. This study is registered with IRCT.ir, number IRCT2017052927520N7.

目的

强直性脊柱炎(AS)是一种炎症性风湿性疾病,炎症主要部位的骨量增加。调节性 T(Treg)细胞已被报道参与 AS 的病理学。本研究旨在研究纳米姜黄素对 AS 患者外周血(PB)中 Treg 细胞反应的影响。

方法

实验组包括 12 例 AS 患者,每日接受纳米姜黄素治疗 4 个月,对照组包括 12 例接受安慰剂的患者。通过流式细胞术测量 Treg 的频率。通过实时聚合酶链反应评估 FoxP3 和几种相关 microRNA(miRNA;miR-27、miR-17 和 miR-146a)以及细胞因子(包括白细胞介素-10(IL-10)、TGF-β和 IL-6)的表达水平。此外,通过酶联免疫吸附试验测定细胞因子的分泌水平。

结果

接受纳米姜黄素治疗后,AS 患者 Treg 细胞的频率显著增加。RT-PCR 数据表明,纳米姜黄素治疗后 miR-17 和 miR-27 的表达明显降低,而 miR-146a 和 FoxP3 的表达明显增加。此外,纳米姜黄素治疗组的 IL-10 和 TGF-β水平较高,IL-6 产生水平较低。

结论

研究结果表明,PB 中 Treg 细胞的失调影响 AS 的发展,纳米姜黄素治疗可能调节 Treg 细胞,因此可能对 AS 及其他自身免疫性疾病的治疗有用。本研究已在 IRCT.ir 注册,编号为 IRCT2017052927520N7。
