TThe aim of this study is to investigate the effect of in vitro cultured Calculus Bovis on the inflammation of oropharynx and body in patients with OSA during the perioperative period of H-UPPP.Eighty patients with OSA and H-UPPP indications were enrolled. The patients were divided into experimental group and control group by random number table, 40 cases in each group. The experimental group was given in vitro cultured Calculus Bovis, while the control group was not given bovine bezoar in vitro. The postoperative oropharyngeal pain, time to resume normal diet, local edema, concentration of IL-1β, IL-8 and TNF-α in saliva, and concentration of IL-1β, IL-8 and TNF-α in blood were compared between the two groups. The pain of oropharynx in the experimental group was lighter than that in the control group on the 3rd, 5th and 7th day after operation (<0.05), but there was no significant difference in the pain of oropharynx between the two groups on the 1st day after operation(>0.05); the time of restoring normal diet in the experimental group was shorter than that in the control group (<0.05); the edema of oropharynx in the experimental group was lighter than that in the control group on the 5th and 7th day after operation (<0.05).The levels of IL-1β, IL-8 and TNF-α in saliva were lower than those in control group on the 3rd, 5th and 7th day after operation (<0.05), and the levels of IL-1β, IL-8 and TNF-α in blood on the 5th and 7th day after operation were lower than those in control group (<0.05).In vitro perioperative period of H-UPPP can improve the postoperative sore throat and local edema of oropharynx, shorten the time of normal diet and reduce the expression of related inflammatory factors in oropharynx and blood.
本研究旨在探讨体外培养牛黄对阻塞性睡眠呼吸暂停(OSA)患者在悬雍垂腭咽成形术(H-UPPP)围手术期口咽及机体炎症的影响。纳入80例有OSA且有H-UPPP适应证的患者。通过随机数字表将患者分为实验组和对照组,每组40例。实验组给予体外培养牛黄,对照组未给予体外牛黄。比较两组术后口咽疼痛、恢复正常饮食时间、局部水肿、唾液中白细胞介素-1β(IL-1β)、白细胞介素-8(IL-8)和肿瘤坏死因子-α(TNF-α)浓度以及血液中IL-1β、IL-8和TNF-α浓度。实验组术后第3天、第5天和第7天口咽疼痛较对照组轻(<0.05),但术后第1天两组口咽疼痛无显著差异(>0.05);实验组恢复正常饮食时间较对照组短(<0.05);实验组术后第5天和第7天口咽水肿较对照组轻(<0.05)。术后第3天、第5天和第7天唾液中IL-1β、IL-8和TNF-α水平低于对照组(<0.05),术后第5天和第7天血液中IL-1β、IL-8和TNF-α水平低于对照组(<0.05)。H-UPPP围手术期体外应用牛黄可改善术后咽痛和口咽局部水肿,缩短正常饮食时间,降低口咽及血液中相关炎症因子的表达。
