Abstract

BACKGROUND AND AIMS

Endoscopic full-thickness resection (EFTR) is an emerging technique for the treatment of various conditions for which classic endoscopic resection techniques have failed or were considered to be at high risk for perforation. The full-thickness resection device (FTRD) is an over-the-scope system which allows a single-step EFTR. The aim of our study is to describe our experience in EFTR of colorectal lesions using the FTRD.

METHODS

Nine patients (10 colorectal lesions) were proposed for EFTR. Safety, R0 resection and endoscopic treatment success were evaluated.

RESULTS

Reasons for referral included nonlifting adenomas ( = 4), nonlifting adenoma recurrence ( = 5), and submucosal lesion ( = 1). EFTR was technically successful in all patients. The mean duration of the procedure was 55 min. R0 resection was obtained in all patients. No major complications were detected. All lesions were successfully treated by the endoscopic technique and no patient was referred for surgery. In patients with available follow-up ( = 6), no recurrence was detected.

CONCLUSIONS

EFTR is a feasible, reasonable time-consuming, safe, and promising endoscopic resection technique.

背景与目的

内镜全层切除术(EFTR)是一种新兴技术,用于治疗经典内镜切除技术失败或被认为穿孔风险高的各种病症。全层切除装置(FTRD)是一种经内镜系统,可实现单步EFTR。我们研究的目的是描述使用FTRD进行大肠病变EFTR的经验。

方法

9例患者(10处大肠病变)接受了EFTR。评估安全性、R0切除及内镜治疗成功率。

结果

转诊原因包括未抬举性腺瘤(4例);未抬举性腺瘤复发(5例);黏膜下病变(1例)。所有患者的EFTR技术均成功。手术平均时长为55分钟。所有患者均实现R0切除。未发现重大并发症。所有病变均通过内镜技术成功治疗,无患者需接受手术治疗。在有随访信息的患者(6例)中,未发现复发情况。

结论

EFTR是一种可行、耗时合理、安全且有前景的内镜切除技术。
