Abstract

OBJECTIVE

To investigate the relations of T lymphocytes, cytokines, immunoglobulin E, and nitric oxide with otitis media with effusion (OME) in children and their clinical significances.

METHODS

Fifty children with OME treated in our hospital were enrolled in the study (observation group). Fifty healthy children were selected as control. The percentages of CD4+ and CD8+ T lymphocyte and CD4+/CD8+ ratio in peripheral blood, and the levels of cytokine (IL)-2, IL-4, IL-6, immunoglobulin E (IgE) and nitric oxide (NO) in peripheral blood and middle ear effusion (MEE) in both groups were detected. The correlations of these indexes with OME were analyzed.

RESULTS

The percentage of peripheral blood CD4+ and CD8+ levels, CD4+/CD8 ratio, IgE, and NO levels in the observation group were significantly higher than those in the control group (P < 0.01). In the observation group, the IL-2 and IL-6 levels, and IgE and NO levels in the MEE were significantly higher than those in peripheral blood (P < 0.01). In addition, in the observation group, the MEE IL-2 and IL-6 levels were positively correlated with peripheral blood CD4+/CD8+ ratio, respectively r = 0.366, P = 0.009; r = 0.334, P = 0.018.

CONCLUSIONS

The levels of peripheral blood CD4+ and CD8+ lymphocytes and MEE IL-2, IL-6, IgE, and NO levels are increased in children with OME. These indexes have provided significant clues for the diagnosis of OME in children.

探讨T淋巴细胞、细胞因子、免疫球蛋白E及一氧化氮与儿童分泌性中耳炎(OME)的关系及其临床意义。

方法

选取我院收治的50例OME患儿作为研究对象(观察组)。选取50例健康儿童作为对照组。检测两组外周血中CD4+和CD8+ T淋巴细胞百分比、CD4+/CD8+比值,以及外周血和中耳积液(MEE)中细胞因子(IL)-2、IL-4、IL-6、免疫球蛋白E(IgE)和一氧化氮(NO)水平。分析这些指标与OME的相关性。

结果

观察组外周血CD4+和CD8+水平百分比、CD4+/CD8比值、IgE及NO水平均显著高于对照组(P < 0.01)。观察组MEE中IL-2和IL-6水平、IgE及NO水平均显著高于外周血(P < 0.01)。此外,观察组MEE中IL-2和IL-6水平分别与外周血CD4+/CD8+比值呈正相关,r = 0.366,P = 0.009;r = 0.334,P = 0.018。

结论

OME患儿外周血CD4+和CD8+淋巴细胞水平以及MEE中IL-2、IL-6、IgE和NO水平升高。这些指标为儿童OME的诊断提供了重要线索。
