Abstract

OBJECTIVE

To observe the therapeutic effect of electroacupuncture (EA) at five Back-Shu points on sleep, hippocampal peripheral benzodiazepine receptor (PBR) expression and hypothalamic 5-hydroxytryptamine (5-HT), 5-hydroxyindoleacetic acid (5-HIAA), tumor necrosis factor alpha (TNF-α) and interleukin (IL)-1β contents in insomnia rats, so as to explore its mechanisms underlying improvement of insomnia.

METHODS

Forty male SD rats were randomly divided into control, model, EA and medication (Diazepam) groups (=10 rats in each group). The insomnia model was established by intraperitoneal injection (i.p.) of para-Chlorophenylalanine (PCPA, 300 mg/kg) once daily for 2 days. EA (60 Hz, 1 mA) was applied to bilateral five Back-Shu points, i.e., Feishu (BL13), Xinshu (BL15), Ganshu (BL18), Pishu (BL20) and Shenshu (BL23) for 10 min, once daily for 6 days. Rats of the medication group were treated by gavage of Diazepam (0.92 mg/kg) once daily for 6 days. The sleep duration was recorded after i.p. of Pentobarbital Sodium (45 mg/kg). Histopathological changes of the hippocampus were displayed by H.E. staining. The contents of 5-HT, 5-HIAA, TNF-α and IL-1β in the hypothalamus were assessed by using ELISA. The expression levels of PBR mRNA and protein in the hippocampus were detected by quantitative real-time PCR, immunohistochemistry and Western blot, separately.

RESULTS

Following modeling, the sleep duration was considerably shortened in rats of the model group relevant to the control group (<0.05). After the treatment, the sleep duration was significantly increased in both EA and medication groups relevant to the model group (<0.05). After modeling, the hippocampal neurons were obviously decreased in number, and disordered in the arrangement, the levels of hypothalamic 5-HT, 5-HIAA, TNF-α, and IL-1β were significantly down-regulated (<0.05), and the expression of PBR mRNA and protein in hippocampus was obviously increased relevant to the control group (<0.05). Following the treatment, the hippocampal neurons were increased in number and arranged regularly, and the contents of hypothalamic 5-HT, 5-HIAA, TNF-α, and IL-1β were considerably increased (<0.05), and the expression of PBR mRNA and protein in hippocampus was significantly decreased in both EA and medication groups relevant to the model group (<0.05). The therapeutic effect of EA was comparable to that of medication in increasing sleep duration, body weight, 5-HT and 5-HIAA contents (>0.05), and significantly superior to that of the medication in increasing TNF-α and IL-1β levels (<0.05), and considerably superior to that of medication in down-regulating PBR mRNA and protein expression (<0.05).

CONCLUSION

EA at five Back-Shu-points of the five Zang-organs can significantly improve the sleep in insomnia rats, which is closely associated with its effects in reducing the expression of PBR in hippocampus and up-regulating the levels of 5-HT, 5-HIAA, TNF-α and IL-1β in hypothalamus.

目的

观察电针五脏背俞穴对失眠大鼠睡眠、海马外周型苯二氮䓬受体(PBR)表达及下丘脑5-羟色胺(5-HT)、5-羟吲哚乙酸(5-HIAA)、肿瘤坏死因子α(TNF-α)和白细胞介素(IL)-1β含量的影响,以探讨其改善失眠的作用机制。

方法

将40只雄性SD大鼠随机分为对照组、模型组、电针组和药物(地西泮)组,每组10只。采用腹腔注射对氯苯丙氨酸(PCPA,300 mg/kg),每日1次,连续2天的方法建立失眠模型。电针组采用60 Hz、1 mA的电流,针刺双侧肺俞(BL13)、心俞(BL15)、肝俞(BL18)、脾俞(BL20)、肾俞(BL23)5个背俞穴,每次10 min,每日1次,连续6天。药物组给予地西泮(0.92 mg/kg)灌胃,每日1次,连续6天。腹腔注射戊巴比妥钠(45 mg/kg)后记录大鼠睡眠时间。采用苏木精-伊红(H.E.)染色观察海马组织病理学变化。采用酶联免疫吸附测定(ELISA)法检测下丘脑5-HT、5-HIAA、TNF-α和IL-1β的含量。分别采用实时荧光定量PCR、免疫组织化学和蛋白质印迹法检测海马组织中PBR mRNA和蛋白的表达水平。

结果

造模后,模型组大鼠睡眠时间较对照组明显缩短(P<0.05)。治疗后,电针组和药物组大鼠睡眠时间较模型组均明显延长(P<0.05)。造模后,模型组海马神经元数量明显减少,排列紊乱,下丘脑5-HT、5-HIAA、TNF-α和IL-1β水平明显下调(P<0.05),海马组织中PBR mRNA和蛋白表达较对照组明显升高(P<0.05)。治疗后,海马神经元数量增多,排列规则,下丘脑5-HT、5-HIAA、TNF-α和IL-1β含量明显升高(P<0.05),电针组和药物组海马组织中PBR mRNA和蛋白表达较模型组均明显降低(P<0.05)。电针组在延长睡眠时间、增加体重、提高5-HT和5-HIAA含量方面与药物组疗效相当(P>0.05),在提高TNF-α和IL-1β水平方面明显优于药物组(P<0.05),在下调PBR mRNA和蛋白表达方面明显优于药物组(P<0.05)。

结论

电针五脏背俞穴可明显改善失眠大鼠的睡眠,其机制可能与降低海马组织中PBR表达,上调下丘脑5-HT、5-HIAA、TNF-α和IL-1β水平有关。
