Abstract

AIM

To evaluate the efficacy and safety of a modified technique [trocar opening (TO)] for silicone oil removal (SOR) in combination with phacoemulsification and intraocular lens (IOL) implantation.

METHODS

A total of 60 eyes of 60 patients with cataract and silicone oil-filled eyes were enrolled in this study. The patients were divided into two groups: the patients in the control group underwent 23-gauge pars plana active SOR surgery with phacoemulsification and IOL implantation, while the patients in the TO group underwent TO methods during surgery. Best corrected visual acuity (BCVA), surgery time, intraocular pressure, and operative complications were observed 6mo after surgery.

RESULTS

There was no significant difference between the two groups in terms of age, gender, preoperative, intraocular pressure, or time of silicone oil stay. Prior to surgery, the mean BCVA for the control and TO groups was 1.34±0.44 and 1.36±0.42. At 6mo following surgery, the mean BCVA improved to 0.74±0.36 and 0.77±0.32, respectively (<0.001). There was no significant difference between the two groups. The mean SOR time was 6.9±2.3min and 4.8±1.2min in the control and TO groups (=0.008). The total operation time was 28.2±8.5min and 24.6±6.4min, respectively (=0.035). Posterior capsule rupture occurred in four eyes of control and none of TO group (<0.01). Late recurrent retinal detachment occurred in one eye in the control group (2mo after surgery) and in one eye in the TO group (4mo after surgery).

CONCLUSION

TO is a simple, effective, time-saving, and safe method for SOR combined with phacoemulsification and IOL implantation.

目的

评估改良技术[套管开口(TO)]用于硅油取出术(SOR)联合超声乳化白内障吸除术及人工晶状体(IOL)植入术的有效性和安全性。

方法

本研究纳入60例白内障合并硅油填充眼患者的60只眼。将患者分为两组:对照组患者接受23G经平坦部主动硅油取出术联合超声乳化白内障吸除术及人工晶状体植入术,而TO组患者在手术中采用TO方法。术后6个月观察最佳矫正视力(BCVA)、手术时间、眼压及手术并发症。

结果

两组患者在年龄、性别、术前眼压或硅油存留时间方面无显著差异。手术前,对照组和TO组的平均BCVA分别为1.34±0.44和1.36±0.42。术后6个月,平均BCVA分别提高至0.74±0.36和0.77±0.32(<0.001)。两组间无显著差异。对照组和TO组的平均硅油取出时间分别为6.9±2.3分钟和4.8±1.2分钟(P=0.008)。总手术时间分别为28.2±8.5分钟和24.6±6.4分钟(P=0.035)。对照组有4只眼发生后囊破裂,TO组无(P<0.01)。对照组有1只眼(术后2个月)和TO组有1只眼(术后4个月)发生晚期复发性视网膜脱离。

结论

TO是一种用于硅油取出术联合超声乳化白内障吸除术及人工晶状体植入术的简单、有效、省时且安全的方法。
