Abstract

BACKGROUND

The present study was conducted to investigate the distribution of virulence factors, capsular serotypes and antibiotic resistance properties of classical (cKP) and hypermucoviscous/hypervirulent (hvKP) isolated from different clinical specimens in Kerman, south-east of Iran.

MATERIALS AND METHODS

A total of 146 isolates were obtained from different clinical specimens. HvKP isolates were identified using the string test. Genes of capsular serotypes K1, K2, K5, K20, K54 and K57 and virulence-associated genes, , , , , , , , and were evaluated by PCR. Antimicrobial susceptibility was also determined using the disc diffusion method.

RESULTS

Out of 146 isolates, 22 (15.1 %) were hvKP. More than half of the hvKP isolates, 13 (59.1%), belonged to non-K1, K2, K5, K20, K54, K57 serotypes. Out of 22 hvKP isolates, 3 and 3 had K1 and K2 serotypes respectively. Among all isolates, 140 (95.9%) and 138 (94.5%) were the most common virulence genes. , and were associated with hvKP isolates (-value <0.05). However, no significant difference was found in , , , and genes between hvKP and cKP strains. HvKP exhibited significantly lower resistance rates to all antimicrobial agents than cKP, except to trimethoprim-sulphamethoxazole and ampicillin (-value <0.05).

CONCLUSION

The frequency of hvKP was low, but overall, the prevalence of virulence-related genes was higher in hvKP than cKP. HvKP was not related to specific serotypes. Furthermore, hvKP isolates were more susceptible to antimicrobial agents compared to cKP isolates.

背景

本研究旨在调查从伊朗东南部克尔曼不同临床标本中分离出的经典肺炎克雷伯菌(cKP)和高黏液/高毒力肺炎克雷伯菌(hvKP)的毒力因子分布、荚膜血清型及抗生素耐药特性。

材料与方法

共从不同临床标本中获得146株分离株。采用拉丝试验鉴定hvKP分离株。通过PCR评估荚膜血清型K1、K2、K5、K20、K54和K57的基因以及毒力相关基因、、、、、、、、和。还采用纸片扩散法测定抗菌药敏性。

结果

在146株分离株中,22株(15.1%)为hvKP。超过一半的hvKP分离株,即13株(59.1%),属于非K1、K2、K5、K20、K54、K57血清型。在22株hvKP分离株中,分别有3株和3株具有K1和K2血清型。在所有分离株中,140株(95.9%)和138株(94.5%)是最常见的毒力基因。、和与hvKP分离株相关(-值<0.05)。然而,在hvKP和cKP菌株之间,、、、和基因未发现显著差异。除对甲氧苄啶-磺胺甲恶唑和氨苄西林外,hvKP对所有抗菌药物的耐药率均显著低于cKP(-值<0.05)。
