To evaluate changes of microvascular network of macular and peripapillary regions and to provide a quantitative measurement of foveal avascular zone (FAZ) in unilateral BRVO patients.
Forty-seven unilateral BRVO patients and forty-seven normal controls were enrolled. A 33 mm scan centered on fovea followed by a 4.54.5 mm scan centered on optic nerve head (ONH) were obtained in BRVO eyes, fellow eyes and control eyes of each individual using OCTA (Optovue Inc., Fremont, CA, USA). Vessel density (VD) in superficial (SVC) and deep vascular complex (DVC) of macula and radial peripapillary capillary (RPC) were automatically calculated. Parameters of FAZ region including size, perimeter, acircularity index (AI) and foveal vessel density 300 (FD-300) were measured.
VDs of SCV and DVC were significantly lower, especially in affected regions, in BRVO eyes compared with fellow eyes (P < 0.05). BRVO affected eyes has larger FAZ size, FAZ perimeter, AI and lower FD-300 compared with fellow eyes (all P < 0.05). VD of SVC and FD-300 were lower in fellow eyes compared with normal control eyes (P < 0.05). The average vessel density in whole area and peripapillary area in BRVO eyes were significantly lower compared with fellow eyes (P < 0.05). VD of inside disc in fellow eyes was lower than normal eyes (P < 0.05).
OCTA provided quantitative information of vascular changes in BRVO. FAZ in BRVO eyes showed significant morphological alterations and decreases of VD in surrounding area. Decreases of VD existed not only in SVC and DVC in macular region but also in RPCs in BRVO eyes. Unaffected eyes of unilateral BRVO showed vascular abnormalities in superficial retinal layer, peri-FAZ area and also peripapillary regions.

纳入 47 例单侧 BRVO 患者和 47 例正常对照者。对 BRVO 眼、对侧眼和每位个体的正常对照眼,分别在黄斑中心进行 33mm 扫描,在视盘中心进行 4.54.5mm 扫描,采用 OCTA(Optovue Inc.,美国 Fremont)进行。自动计算黄斑区浅层血管丛(SVC)和深层血管丛(DVC)以及视盘周围放射状毛细血管(RPC)的血管密度(VD)。测量 FAZ 区域的大小、周长、非圆度指数(AI)和 FAZ 中心 300μm 处的血管密度(FD-300)等参数。
与对侧眼相比,BRVO 眼的 SVC 和 DVC 的 VD 明显较低,特别是在病变区域(P<0.05)。BRVO 患眼的 FAZ 大小、FAZ 周长、AI 均大于对侧眼,FD-300 低于对侧眼(均 P<0.05)。与正常对照组相比,对侧眼的 SVC 和 FD-300 较低(P<0.05)。BRVO 眼的整个区域和视盘周围区域的平均血管密度明显低于对侧眼(P<0.05)。对侧眼的视盘内区的 VD 低于正常眼(P<0.05)。
OCTA 提供了 BRVO 血管变化的定量信息。BRVO 眼的 FAZ 显示出明显的形态改变和周围区域 VD 的减少。不仅在黄斑区的 SVC 和 DVC,而且在 BRVO 眼的 RPC 中,均存在 VD 减少。单侧 BRVO 的未受影响眼在浅层视网膜层、FAZ 周围区域和视盘周围区域存在血管异常。
