Abstract

OBJECTIVES

This study aims to evaluate the effects of additional low-level laser therapy (940 nm GaAlAs) to the routine occlusal splint therapy on maximal mouth opening (MMO), visual analog scale (VAS) scores, and passive mouth opening (PMO).

PATIENTS AND METHODS

Between January 2016 and March 2018, a total of 40 patients (10 males, 30 females; mean age 22.35 years; range, 18 to 60 years) with disc displacement with reduction of the temporomandibular joint were included. The patients were divided into two groups. Group 1 (control group, n=20) received routine non-steroidal anti-inflammatory drug (NSAID) therapy and occlusal splint therapy for eight hours per day for a total of three months. Group 2 (laser group, n=20) received NSAID, occlusal splint therapy and 940 nm wavelength diode laser therapy for two sessions per week for a total of four weeks. The MMO, PMO, and VAS scores were recorded before and at one, two, three, four, and 12 weeks after treatment. The joint and muscle examinations were performed.

RESULTS

A constant decrease was found in the VAS scores in both groups. At four (p=0.008) and 12 weeks (p=0.003), a statistically significant difference in the VAS scores was observed. At all time points, PMO and MMO scores increased in both groups. The MMO scores were statistically higher in the laser group at all time points (p<0.005).

CONCLUSION

Our study results suggest that short-term low-level laser therapy additional to conventional treatment may yield satisfactory outcomes.

目的

本研究旨在评估在常规牙合垫治疗基础上加用低强度激光治疗(940nm 砷化镓铝激光)对最大开口度(MMO)、视觉模拟评分(VAS)以及被动开口度(PMO)的影响。

患者与方法

在 2016 年 1 月至 2018 年 3 月期间,共纳入 40 例颞下颌关节盘可复性移位患者(男性 10 例,女性 30 例;平均年龄 22.35 岁;年龄范围 18 至 60 岁)。将患者分为两组。第 1 组(对照组,n = 20)接受常规非甾体抗炎药(NSAID)治疗以及每天佩戴牙合垫 8 小时,共持续三个月。第 2 组(激光组,n = 20)接受 NSAID、牙合垫治疗以及波长为 940nm 的二极管激光治疗,每周两次,共持续四周。记录治疗前以及治疗后 1、2、3、4 和 12 周时的 MMO、PMO 和 VAS 评分。同时进行关节和肌肉检查。

结果

两组的 VAS 评分均持续下降。在第 4 周(p = 0.008)和第 12 周(p = 0.003)时,VAS 评分存在统计学显著差异。在所有时间点,两组的 PMO 和 MMO 评分均升高。激光组在所有时间点的 MMO 评分均显著高于对照组(p < 0.005)。

结论

我们的研究结果表明,在传统治疗基础上短期加用低强度激光治疗可能会产生满意的效果。
