The effect of physical therapy and rehabilitation modalities on sleep quality in patients with primary knee osteoarthritis: A single-blind, prospective, randomized-controlled study.

Abstract

OBJECTIVES

In this study, we aimed to evaluate the effect of physical therapy modalities on pain, sleep, mental status, and quality of life of patients with osteoarthritis (OA).

PATIENTS AND METHODS

Between January 2017 and June 2017, a total of 40 patients (9 males, 31 females; mean age 56.6±8.9 years; range, 40 to 70 years) who were diagnosed with knee OA according to the American College of Rheumatology (ACR) criteria and were in Kellgren- Lawrence Grade 2-3 were included in the study. The patients were divided into two groups equally. Both groups received 15 sessions of exercise therapy, whereas the intervention group also received 10 consecutive physical therapy sessions in the form of hot pack, therapeutic ultrasound (US), and transcutaneous electrical nerve stimulation (TENS) by a single physiotherapist. Isometric and isotonic exercises were planned as 10 reps for three times a day as a home-based program. Clinical assessments were performed using the visual analog scale (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Beck Depression Inventory (BDI), Pittsburg Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), and Short Form 36 (SF-36) before and after treatment.

RESULTS

Pre-treatment VAS, ESS, PQSI, BDI, WOMAC, and SF-36 scores showed no significant difference between the groups, whereas post-treatment scores showed a significant difference in the intervention group (p<0.05). The difference between the pre- and post-treatment VAS, ESS, PQSI, BDI, WOMAC, and SF-36 scores were significantly higher in the intervention group, compared to the controls (p<0.05).

CONCLUSION

Our study results show positive effects of exercise and physical therapy modalities on pain, disease activity, sleep quality, depression, and quality of life in knee OA patients.

目的

在本研究中,我们旨在评估物理治疗方式对骨关节炎(OA)患者的疼痛、睡眠、心理状态和生活质量的影响。

患者与方法

在2017年1月至2017年6月期间,共有40例患者(9例男性,31例女性;平均年龄56.6±8.9岁;范围为40至70岁)被纳入研究,这些患者根据美国风湿病学会(ACR)标准被诊断为膝关节OA,且处于Kellgren-Lawrence 2-3级。患者被平均分为两组。两组均接受15节运动疗法课程,而干预组还由一名物理治疗师以热敷、治疗性超声(US)和经皮电刺激神经疗法(TENS)的形式连续接受10节物理治疗课程。等长和等张运动计划作为家庭计划,每天进行3次,每次10次重复。在治疗前后使用视觉模拟量表(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、贝克抑郁量表(BDI)、匹兹堡睡眠质量指数(PSQI)、爱泼华嗜睡量表(ESS)和简明健康调查问卷(SF-36)进行临床评估。

结果

治疗前VAS、ESS、PQSI、BDI、WOMAC和SF-36评分在两组之间无显著差异,而治疗后评分在干预组中有显著差异(p<0.05)。与对照组相比,干预组治疗前后VAS、ESS、PQSI、BDI、WOMAC和SF-36评分的差异显著更高(p<0.05)。

结论

我们的研究结果表明运动和物理治疗方式对膝关节OA患者的疼痛、疾病活动度、睡眠质量、抑郁和生活质量有积极影响。
