Subthalamic Nucleus Deep Brain Stimulation in Primary Meige Syndrome: A 1-Year Follow-Up Study.

Abstract

OBJECTIVE

To investigate the efficacy of bilateral subthalamic nucleus (STN) deep brain stimulation (DBS) in patients with Meige syndrome.

MATERIALS AND METHODS

Fifteen consecutive patients who underwent STN-DBS at the Peking University People's Hospital between September 2017 and June 2018 were included in this study. The Burke-Fahn-Marsden Dystonia Rating Scale (BFMDRS) movement score and the BFMDRS disability score were obtained prior to surgery, and at specific time points after surgery. Patients' sleep status was also assessed before and after surgery.

RESULTS

The BFMDRS movement scores decreased from 15.3 ± 4.6 to 5.2 ± 6.2 after STN-DBS, with a mean improvement of 68.6% (p < 0.05). The BFMDRS disability scores were also significantly decreased, from 6.9 ± 3.3 to 3.5 ± 2.9, with a mean improvement of 51.7% (p < 0.05). The eye, mouth, speech, and swallowing movement scores also decreased significantly after STN-DBS compared to baseline (p < 0.05). The sleep quality of the patients was also improved after surgery.

CONCLUSIONS

These findings demonstrate that the STN is an effective brain target for the treatment of patients with Meige syndrome. STN-DBS was not only able to improve patients' motor symptoms, but also their sleep status.

目的

研究双侧丘脑底核(STN)深部脑刺激(DBS)治疗Meige 综合征的疗效。

材料与方法

本研究纳入了 2017 年 9 月至 2018 年 6 月期间在北京大学人民医院接受 STN-DBS 的 15 例连续患者。手术前和手术后特定时间点分别采用 Burke-Fahn-Marsden 肌张力障碍评定量表(BFMDRS)运动评分和 BFMDRS 残疾评分进行评估。同时,还在手术前后评估了患者的睡眠状态。

结果

STN-DBS 后,BFMDRS 运动评分从 15.3±4.6 降至 5.2±6.2,平均改善 68.6%(p<0.05)。BFMDRS 残疾评分也显著降低,从 6.9±3.3 降至 3.5±2.9,平均改善 51.7%(p<0.05)。与基线相比,STN-DBS 后眼部、口腔、言语和吞咽运动评分也显著降低(p<0.05)。术后患者的睡眠质量也得到改善。

结论

这些发现表明 STN 是治疗 Meige 综合征患者的有效脑靶点。STN-DBS 不仅能够改善患者的运动症状,还能改善其睡眠状态。
