The integration of paediatric palliative care into the Iranian health system is essential.
The aim of this study was to identify the challenges of palliative care in the paediatric intensive care unit during COVID-19 through the experiences of healthcare providers.
A qualitative study with content analysis approach was conducted. Fifteen physicians and nurses were selected by purposeful sampling. The semi-structured, in-depth interviews were applied in the data collection.
Ten main categories were extracted from data analysis, including 'caring in COVID-19', 'communication and family centre care', 'breaking bad news', palliative care training', 'pain and symptom management', 'support of the child, family and clinical team', 'physical environment', 'guidelines', 'specialised staff' and 'home based palliative care'.
Palliative care in the PICU faces several challenges, especially during COVID-19, but the clinical team are making every attempt to improve the comprehensive care of children and their families. Telehealth is important in COVID-19, and education is also a key component to improve palliative care in the PICU in Iran.
将儿科姑息治疗纳入伊朗卫生系统至关重要。
本研究旨在通过医疗服务提供者的经验，确定新冠疫情期间儿科重症监护病房姑息治疗面临的挑战。
采用内容分析法进行定性研究。通过目的抽样选取了15名医生和护士。数据收集采用半结构化深度访谈。
数据分析提取出十个主要类别，包括“新冠疫情期间的护理”“沟通与以家庭为中心的护理”“告知坏消息”“姑息治疗培训”“疼痛和症状管理”“对儿童、家庭和临床团队的支持”“物理环境”“指南”“专业人员”和“居家姑息治疗”。
儿科重症监护病房的姑息治疗面临若干挑战，尤其是在新冠疫情期间，但临床团队正尽一切努力改善对儿童及其家庭的综合护理。远程医疗在新冠疫情期间很重要，教育也是改善伊朗儿科重症监护病房姑息治疗的关键组成部分。
