Abstract

This study explored the predictive value of postoperative C-reactive protein in children with hypospadias for postoperative complications and the risk factors. The clinical and follow-up data of 106 children with hypospadias who were treated with operations at Kunming Children's Hospital in 2020 were, respectively, analyzed. According to the occurrence of postoperative complications, the patients were divided into two groups: 25 patients with postoperative complications were the complications group, and 81 without postoperative complications were the control group. The baseline data, clinical characteristics, laboratory test indexes, and outcome of the two groups were collected. Receiver operating characteristic (ROC) was used to calculate the optimal cutoff value of C-reaction protein (CRP). Logistic regression was used to analyze the risk factors of hypospadias after surgery. A probability value () < 0.05 was considered statistically significant. According to the result of the ROC curve, the optimal cutoff value of CRP was 11.7 mg/L. Logistic regression showed that the length of urethral defect, the urethral material, the operative produce, and the postoperative CRP level were related to the occurrence of postoperative complications of patients with DCC. The length of the urethral defect and the CRP level were the independent risk factors of the prognosis of hypospadias patients. The CRP level was related to the occurrence of postoperative complications and fistula. Postoperative CRP level can be used as a reliable marker for predicting the prognosis of hypospadias patients.

本研究探讨了尿道下裂患儿术后C反应蛋白对术后并发症的预测价值及危险因素。对2020年在昆明市儿童医院接受手术治疗的106例尿道下裂患儿的临床及随访资料进行了分析。根据术后并发症的发生情况,将患者分为两组:25例发生术后并发症的患者为并发症组,81例未发生术后并发症的患者为对照组。收集两组的基线资料、临床特征、实验室检查指标及结局。采用受试者工作特征(ROC)曲线计算C反应蛋白(CRP)的最佳截断值。采用Logistic回归分析尿道下裂术后的危险因素。概率值(P)<0.05认为差异有统计学意义。根据ROC曲线结果,CRP的最佳截断值为11.7mg/L。Logistic回归分析显示,尿道缺损长度、尿道材料、手术方式及术后CRP水平与DCC患者术后并发症的发生有关。尿道缺损长度及CRP水平是尿道下裂患者预后的独立危险因素。CRP水平与术后并发症及瘘管的发生有关。术后CRP水平可作为预测尿道下裂患者预后的可靠指标。
