Abstract

BACKGROUND

Previous studies have mostly explored the comorbidities of Global developmental delay (GDD) in children with Autism Spectrum Disorders (ASD) from the perspective of ASD. The study focus on the perspective of GDD to investigate the prevalence and developmental profiles of ASD in GDD and to explore the correlation between the developmental level and symptoms of autism.

METHODS

Clinical data of 521 children with GDD aged from 24 to 60 months were retrospectively analyzed. Analyses were performed first for the whole sample and then subdivided into two subgroups (GDDASD, GDDASD) according to whether had ASD. Symptoms of autism were evaluated by the Autism Behavior Checklist and the Childhood Autism Rating Scale. The Chinese version of the Gesell Developmental Schedules was used to evaluate the level of children's mental development.

RESULT

The prevalence of ASD in children with GDD was 62.3%. The total average developmental quotient (DQ) of GDD was mildly deficient and was negatively correlated with symptoms of autism ( < 0.05); language ability was severe and extremely severe deficient ( < 0.05). GDDASD group and GDDASD group have some common points as well as differences in the developmental features. The language delay of children in both subgroups was the most obviously defected, thereafter followed by the item of personal social activity. In the GDDASD group, the DQ of gross motor skills>fine motor skills>adaptability ( < 0.05). There were no significant differences among the DQ of gross motor skills, fine motor skills and adaptability in GDDASD group ( > 0.05). The GDDASDgroup had better adaptability, fine motor skills, language ability, personal social activity than that of the GDDASD group, but the gross motor skills in GDDASD group were worse ( < 0.05).

CONCLUSION

GDD children have a high proportion of comorbid ASD, and GDD children with poorer developmental levels are more likely to have ASD symptoms. Development profiles in both GDDASD children and GDDASD children have common features but there are also differences. GDDASD group is worse than GDDASD group in terms of the overall development level.

方法

回顾性分析521例年龄在24至60个月的GDD儿童的临床资料。首先对整个样本进行分析,然后根据是否患有ASD分为两个亚组(GDD伴ASD、GDD不伴ASD)。采用孤独症行为量表和儿童孤独症评定量表评估孤独症症状。使用中文版格塞尔发育量表评估儿童智力发育水平。

结果

GDD儿童中ASD的患病率为62.3%。GDD的总平均发育商(DQ)轻度缺陷,且与孤独症症状呈负相关(P<0.05);语言能力严重和极严重缺陷(P<0.05)。GDD伴ASD组和GDD不伴ASD组在发育特征上有一些共同点也有差异。两个亚组儿童的语言发育迟缓最为明显,其次是个人社交活动项目。在GDD伴ASD组中,大运动技能DQ>精细运动技能DQ>适应性DQ(P<0.05)。GDD不伴ASD组的大运动技能DQ、精细运动技能DQ和适应性DQ之间无显著差异(P>0.05)。GDD不伴ASD组在适应性、精细运动技能、语言能力、个人社交活动方面比GDD伴ASD组更好,但GDD伴ASD组的大运动技能更差(P<0.05)。

结论

GDD儿童中ASD共病比例高,发育水平较差的GDD儿童更易出现ASD症状。GDD伴ASD儿童和GDD不伴ASD儿童的发育概况有共同特征但也有差异。GDD伴ASD组在总体发育水平上比GDD不伴ASD组差。
