Abstract

BACKGROUND

Recent global evidences showed that asymptomatic blood donor carriers of Leishmania infection will appear as a threat for blood transfusions recipients in endemic areas. As yet, there is no appropriate diagnostic procedure for detecting infection of blood donors in blood banks.

SUBJECTS AND METHODS

The present study was aimed to apply various current diagnostic tests among blood donors in an endemic area of visceral leishmaniasis (VL), Ardabil Province, northwestern Iran. Blood samples were gathered from 860 blood donors in endemic areas of the province between 2017 and 2018, at eight blood donation centers. These samples was assessed using microculture, serological (DAT and rK39-ICT) and molecular based (conventional kDNA-PCR and HRM-PCR) tests.

RESULTS

Of 860 eligible donors, 24 (2.8%) were seropositive for VL by DAT, and 388 (45%) were positive by kDNA-PCR. Moreover, 19 (19/860) were positive for both of them. Out of 19 subjects, 5.3% (1/19) was positive by rK39-ICT, 10.5% (2/19), and 79% (15/19) were detected positive in microculture and HRM-PCR methods, respectively. Nineteen donors were followed up for 2 years, of which 16 (84.2%) had a serological conversion, and 4 (21%) were positive by kDNA-PCR. The sensitivity of kDNA-PCR, and HRM-PCR procedures in detecting Leishmania parasite was found to be 98.7%, and 79%, respectively.

CONCLUSIONS

Our findings justify the use of kDNA-PCR as a convenient and sensitive tool for screening subjects with leishmanial latent infection in blood banks at least in endemic regions. In these areas, however, a PCR-based test should be used to validate Leishmania infection among seropositive donors.

本研究旨在对伊朗西北部阿尔达比勒省内脏利什曼病(VL)流行地区的 860 名献血者应用各种当前的诊断检测方法。2017 年至 2018 年期间,在该省八个献血中心采集了来自流行地区的 860 名献血者的血样。这些样本使用微培养、血清学(DAT 和 rK39-ICT)和分子检测(常规 kDNA-PCR 和 HRM-PCR)进行评估。

结果

在 860 名合格献血者中,24 名(2.8%)DAT 血清学阳性,388 名(45%)kDNA-PCR 阳性。此外,有 19 名(19/860)两种方法均为阳性。在这 19 名患者中,5.3%(1/19)rK39-ICT 阳性,10.5%(2/19),79%(15/19)通过微培养和 HRM-PCR 方法检测阳性。19 名献血者随访 2 年,其中 16 名(84.2%)出现血清学转换,4 名(21%)kDNA-PCR 阳性。kDNA-PCR 和 HRM-PCR 检测利什曼原虫的灵敏度分别为 98.7%和 79%。

结论

我们的研究结果证明,kDNA-PCR 作为一种方便、敏感的工具,至少在流行地区的血库中用于筛查具有利什曼原虫潜伏感染的人群是合理的。然而,在这些地区,应使用基于 PCR 的检测方法来验证血清学阳性献血者中的利什曼感染。
