To compare the application value of dynamic enhanced magnetic resonance imaging (MRI) and ultrasonic diffused optical tomography (DOT) in early diagnosis of breast cancer.
The clinical data of 110 female patients with breast diseases treated in our hospital from June 2018 to June 2021 were selected for the retrospective analysis, and the patients were divided into the benign lesion group ( = 50) and breast cancer group ( = 60) according to the pathologic findings. All patients received dynamic enhanced MRI and ultrasonic DOT examinations for the observation of lesion morphology and analysis of relevant parameters, so as to scientifically evaluate the diagnostic value of dynamic enhanced MRI and ultrasonic DOT for early breast cancer.
The dynamic enhanced MRI examination found that the proportions of irregular shape, increased vascular shadow, obscure boundary, spicule sign, heterogeneous enhancement, etc. of lesion were significantly higher in the breast cancer group than in the benign lesion group ( < 0.05); parameters such as , , and were significantly higher in the breast cancer group than in the benign lesion group ( < 0.05); the ultrasonic DOT diagnosis found that the THC value was obviously lower in the benign lesion group than in the breast cancer group ( < 0.05); compared with the pathologic findings, it was believed that combined diagnosis had significantly higher diagnosis accuracy rate, sensitivity, specificity, positive predictive value and negative predictive value than the dynamic enhanced MRI and ultrasonic DOT diagnosis alone ( < 0.05); and after further analyzing the efficacy of the two diagnosis modalities in diagnosing early breast cancer by ROC curves, the result showed combined diagnosis > dynamic enhanced MRI > ultrasonic DOT.
Both dynamic enhanced MRI and ultrasonic DOT present higher diagnostic value to early breast cancer, of which dynamic enhanced MRI obtains results closer to the pathologic findings and has diagnostic efficacy higher than ultrasonic DOT. But the combination of the two can significantly improve the diagnosis accuracy rate for early breast cancer, presenting higher diagnostic value.
比较动态增强磁共振成像(MRI)和超声扩散光学断层成像(DOT)在乳腺癌早期诊断中的应用价值。
回顾性分析 2018 年 6 月至 2021 年 6 月我院收治的 110 例女性乳腺疾病患者的临床资料,根据病理结果将患者分为良性病变组(n=50)和乳腺癌组(n=60)。所有患者均接受动态增强 MRI 和超声 DOT 检查,观察病灶形态并分析相关参数,科学评价动态增强 MRI 和超声 DOT 对早期乳腺癌的诊断价值。
动态增强 MRI 检查发现,乳腺癌组病灶形态不规则、血管影增多、边界模糊、毛刺征、不均匀强化等比例明显高于良性病变组(<0.05);乳腺癌组 、 、 等参数明显高于良性病变组(<0.05);超声 DOT 诊断发现良性病变组 THC 值明显低于乳腺癌组(<0.05);与病理结果相比,联合诊断的诊断准确率、敏感度、特异度、阳性预测值和阴性预测值均明显高于单纯动态增强 MRI 和超声 DOT 诊断(<0.05);进一步通过 ROC 曲线分析两种诊断方法诊断早期乳腺癌的效果,结果显示联合诊断>动态增强 MRI>超声 DOT。
动态增强 MRI 和超声 DOT 对早期乳腺癌均具有较高的诊断价值,其中动态增强 MRI 与病理结果更接近,诊断效能优于超声 DOT,但两者联合可明显提高早期乳腺癌的诊断准确率,具有更高的诊断价值。
