The evolution of the COVID-19 pandemic affected healthcare systems worldwide. The patients with chronic kidney disease (CKD), diabetes, and cardiovascular disease were most affected and had an unfavorable outcome.
We examined the activity of the Nephrology Department from Târgu-Mureș County Emergency Hospital retrospectively, comparing two periods: June 2020-November 2021 (COVID-19 period) and June 2018-November 2019 (non-COVID-19 period).
In the COVID-19 period, there were fewer one-day hospitalizations registered, 77.27% more dialysis catheters were installed, and 43.75% more arteriovenous fistulas were performed. An overall increase in the number of patients requiring dialysis during the pandemic was recorded, as of the number of dialysis sessions performed. Moreover, we observed a statistically significant increase in the number of dialysis sessions per patient and a statistically significant increase in the number of hospitalization days in the pandemic interval. Acute kidney injury (AKI) and urosepsis were the diagnoses that increased the most among in-patients during the pandemic, while all other nephrology diagnoses decreased.
The COVID-19 pandemic accelerated kidney pathology and worsened the outcomes of nephrology patients in our center. The number of chronic and patient's access to one-day hospitalization decreased in order to minimalize the exposure and the risk of infection. In contrast, the need for emergency dialysis increased significantly.
新冠疫情的演变影响了全球医疗系统。慢性肾脏病(CKD)、糖尿病和心血管疾病患者受影响最大,且预后不佳。
我们回顾性研究了特尔古穆列什县急诊医院肾内科的活动,比较了两个时期:2020年6月至2021年11月(新冠疫情时期)和2018年6月至2019年11月(非新冠疫情时期)。
在新冠疫情时期,单日住院登记人数减少,透析导管置入量增加77.27%,动静脉内瘘手术量增加43.75%。记录显示,疫情期间需要透析的患者总数以及透析治疗次数总体增加。此外,我们观察到每位患者的透析治疗次数有统计学意义的增加,且疫情期间住院天数有统计学意义的增加。急性肾损伤(AKI)和尿脓毒症是疫情期间住院患者中增加最多的诊断,而所有其他肾病诊断均减少。
新冠疫情加速了肾脏病变,恶化了我们中心肾病患者的预后。为尽量减少暴露和感染风险,慢性患者数量及一日住院患者数量减少。相比之下,紧急透析需求显著增加。
