To explore the relationship between high density lipoprotein (HDL) and lower extremities deep venous thrombosis (DVT) in patients undergoing hip arthroplasty.
A total of 348 patients undergoing hip arthroplasty in our hospital were enrolled, and divided into observation ( = 154, 44.25%) and control ( = 194, 55.75%) groups according to the occurrence of lower extremities DVT. The presence of DVT was assessed 1 day before surgery and routinely every 2 days after surgery. The factors of DVT were analyzed by single factor analysis, multivariate logistic regression analysis, and Pearson correlation.
The age and body mass index in the observation group were significantly higher ( = .045, = .041, respectively), while HDL-C was significantly lower ( = .032) than the control group. Increase age, high BMI, low apolipoprotein-A1 level and low HDL-C level were risk factors for lower extremities DVT. The mean HDL-C in the observation and control groups was 0.91 ± 0.27 and 1.19 ± 0.37, respectively, the adjusted odds ratio was 1.050; 95% CI 1.010-1.092, = .014.
Elderly patients with high BMI and low HDL-C level undergoing hip arthroplasty are at risk of lower extremities DVT, and should be paid attention to clinically.
探讨髋部置换术患者高密度脂蛋白(HDL)与下肢深静脉血栓(DVT)的关系。
选取我院收治的 348 例行髋部置换术患者,根据下肢 DVT 的发生情况分为观察组(n=154,44.25%)和对照组(n=194,55.75%)。术前 1 天及术后每 2 天常规评估 DVT 发生情况。采用单因素分析、多因素 logistic 回归分析和 Pearson 相关性分析下肢 DVT 的影响因素。
观察组年龄和 BMI 显著高于对照组( =.045, =.041),而 HDL-C 显著低于对照组( =.032)。高龄、高 BMI、载脂蛋白 A1 水平低和 HDL-C 水平低是下肢 DVT 的危险因素。观察组和对照组的平均 HDL-C 分别为 0.91 ± 0.27 和 1.19 ± 0.37,调整后的优势比为 1.050;95%CI 为 1.010-1.092, =.014。
髋部置换术的老年患者 BMI 高且 HDL-C 水平低,下肢 DVT 风险增加,临床应予以重视。
