Influence of Work Values on the Prescribing Behavioral Intentions Regarding Antibiotic Use Among Primary Physicians in Hubei, China.

Abstract

OBJECTIVE

Primary physicians have been an important cause of global antibiotic resistance. The aim of this study is to identify the influence of primary physicians' work values on prescribing behavioral intentions regarding antibiotic use (behavioral intentions).

METHODS

A total of 656 primary physicians' work values and behavioral intentions were collected by a stratified cluster sampling from 67 primary care facilities in Hubei Province of China. Work values included 5 dimensions, namely intrinsic values, extrinsic values, reward values, social values and altruistic values. Behavioral intentions included 2 dimensions of the intentions to reduce antibiotic prescriptions and the intentions to prescribe antibiotics. A Likert five-point scale was used and higher scores meant greater intentions to prescribe antibiotics. A hierarchical multiple regression analysis was employed to model the influence of work values on behavioral intentions.

RESULTS

Primary physicians' behavioral intention was 2.01 averagely. Intrinsic values negatively influenced overall intentions to prescribe more antibiotics (β = -0.098, = 0.010). Whereas lower social values perception (β = 0.248, < 0.001), less pursuit of reward values (β = 0.194, < 0.001), and less emphasis on altruistic values (β = 0.180, < 0.001) positively influenced lower overall intentions to prescribe antibiotic prescriptions. Besides, extrinsic values were not found influencing the behavioral intentions (β = 0.001, = 0.961).

CONCLUSIONS

Primary physicians' work values influenced their behavioral intentions regarding antibiotic use. Training and education of work values may be an entry point for intervention on improving antibiotic prescribing.

目的

初级保健医生是导致全球抗生素耐药性的一个重要原因。本研究旨在确定初级保健医生的工作价值观对其开出处方行为意向(行为意向)的影响。

方法

采用分层整群抽样方法,从中国湖北省的 67 家基层医疗保健机构中收集了 656 名初级保健医生的工作价值观和行为意向。工作价值观包括内在价值、外在价值、奖励价值、社会价值和利他主义价值 5 个维度。行为意向包括减少抗生素处方和开抗生素处方的意向 2 个维度。采用 5 点 Likert 量表,得分越高表示开抗生素处方的意向越强。采用分层多元回归分析模型来分析工作价值观对行为意向的影响。

结果

初级保健医生的行为意向平均得分为 2.01。内在价值观对总体开更多抗生素的意向有负向影响(β=-0.098, =0.010)。而较低的社会价值观感知(β=0.248, <0.001)、较少追求奖励价值(β=0.194, <0.001)和较少强调利他主义价值(β=0.180, <0.001),则正向影响总体上减少抗生素处方的意向。此外,外在价值观并不影响行为意向(β=0.001, =0.961)。

结论

初级保健医生的工作价值观影响其对抗生素使用的行为意向。工作价值观的培训和教育可能是改善抗生素处方的干预切入点。
