Abstract

Common variable immunodeficiency (CVID) or acquired hypogammaglobulinemia is one of the common forms of primary immunodeficiency, which primarily affects the respiratory tract, but is often associated with gastrointestinal complications. The pathophysiology is not fully understood, making a diagnosis of CVID difficult. The low levels of IgG and IgA and defective B cells make intravenous immunoglobulin (IVIG) therapy the mainstay of management. The present article describes the journey of a 41-year-old male suffering from CVID and severe recurrent uncontrollable gastrointestinal infections requiring Intravenous immunoglobulin therapy, which was successfully substituted with an alternative oral Hyperimmune Bovine Colostrum and Zinc combination for long term gastrointestinal disease control in a more affordable manner. 摘要

普通可变免疫缺陷（CVID）或获得性低丙种球蛋白血症是原发性免疫缺陷的常见形式之一，主要影响呼吸道，但常伴有胃肠道并发症。其病理生理学尚未完全明确，这使得CVID的诊断较为困难。IgG和IgA水平低下以及B细胞缺陷使得静脉注射免疫球蛋白（IVIG）治疗成为主要的治疗手段。本文描述了一名41岁男性患CVID且严重反复出现无法控制的胃肠道感染，需要静脉注射免疫球蛋白治疗的历程，最终成功地用一种替代性的口服高免疫牛初乳和锌组合以更经济实惠的方式替代了该治疗，用于长期控制胃肠道疾病。关键词：普通可变免疫缺陷、低丙种球蛋白血症、原发性免疫缺陷、胃肠道感染、静脉注射免疫球蛋白治疗、牛初乳
