Congenital muscular torticollis (CMT) is a common musculoskeletal anomaly that can be excellently managed at birth with conservative physiotherapy; as a result, literature on neglected cases of CMT in older patients is sparse, and there is controversy regarding the ideal surgical approach. This report aims to provide the outcome of 28 adolescent patients with neglected CMT who underwent unipolar release at the distal sternocleidomastoid muscle (SCM).
Twenty-eight adolescent patients with untreated CMT presented to our department between 2016 and 2019 and underwent unipolar release at the distal end of their SCM. All patients had no other anomaly based on clinical examination and radiological investigations. At the time of surgery, the mean age of patients was 15.8 years (range 13-18 years). There were 10 male and 18 female patients with the right side affected in 20 cases. All patients were followed up for 2 years. Patients were evaluated using an adapted version of the modified Lee's scoring system to assess cosmesis and the cervical-mandibular angle (CMA) to assess radiological change.
The adapted modified Lee's scoring system indicated 17 patients (60.7%) had an excellent outcome, 6 patients (21.4%) had a good outcome, and 5 patients (17.9%) had a fair outcome. In particular, scarring was fine in 23 patients (82.1%) and only slight in the remaining. Independent to the categorical outcome, all patients subjectively reported high satisfaction with their cosmesis. The preoperative mean CMA was 19.6° (range, 8.5-31.5°), which was reduced to a mean of CMA of 14.0° (range, 3-28°) after surgery ( < 0.05). No patient developed any permanent complication or required surgery for recurrence. No serious postoperative complications such as infection or hematoma were observed.
In uncomplicated cases of neglected CMT in adolescent patients, unipolar tenotomy of the distal SCM is a safe and reliable technique with good clinical outcomes.
先天性肌性斜颈(CMT)是一种常见的肌肉骨骼异常疾病,出生时采用保守物理治疗可取得良好效果;因此,关于老年患者中被忽视的CMT病例的文献较少,且对于理想的手术方法存在争议。本报告旨在提供28例接受胸锁乳突肌(SCM)远端单极松解术的青少年被忽视CMT患者的治疗结果。
2016年至2019年间,28例未经治疗的CMT青少年患者到我院就诊,并接受了SCM远端单极松解术。根据临床检查和影像学检查,所有患者均无其他异常。手术时,患者的平均年龄为15.8岁(范围13 - 18岁)。其中男性10例,女性18例,右侧受累20例。所有患者均随访2年。采用改良Lee评分系统的改编版评估患者的美容效果,采用颈下颌角(CMA)评估放射学变化。
改编后的改良Lee评分系统显示,17例患者(60.7%)效果极佳,6例患者(21.4%)效果良好,5例患者(17.9%)效果尚可。具体而言,23例患者(82.1%)瘢痕良好,其余患者瘢痕轻微。不考虑分类结果,所有患者主观上对其美容效果满意度较高。术前平均CMA为19.6°(范围8.5 - 31.5°),术后平均CMA降至14.0°(范围3 - 28°)(<0.05)。没有患者出现任何永久性并发症或因复发需要再次手术。未观察到严重的术后并发症,如感染或血肿。
对于青少年患者中未合并其他复杂情况的被忽视CMT病例,SCM远端单极腱切断术是一种安全可靠的技术,临床效果良好。
