Abstract

OBJECTIVE

To evaluate the effect of metformin combined with intensive-exercise diet therapy on glucose and lipid metabolism and islet function in diabetes patients with localized renal cell carcinoma after laparoscopic resection.

METHODS

A total of 120 renal cancer patients with diabetes mellitus treated in the oncology department of our hospital from January 2018 to December 2020 were recruited and assigned via random number table method at a ratio of 1 : 1 to receive either metformin (control group) or metformin plus intensive exercise diet therapy (study group) after laparoscopic nephrectomy. Outcome measures included glucose and lipid metabolism, pancreatic islet function, lifestyle, clinical efficacy, and adverse reactions.

RESULTS

After the intervention, the fasting blood glucose (FBG), 2 h postprandial blood glucose (PBG), glycosylated hemoglobin (HbA1c), triglyceride (TG), total cholesterol (TC), and low-density lipoprotein cholesterol (LDL-C) of the two groups of patients decreased significantly, and the study group had significantly lower results. After treatment, the two groups had elevated levels of high-density lipoprotein cholesterol (HDL-C), fasting serum insulin (FINS), and homeostasis model assessment of -cell function (HOMA-), and higher results were obtained in the study group ( < 0.05). After the intervention, the study group showed higher results of health promoting lifestyle profile-II (HPLP-II) and a 12-month progression-free survival rate than the control group. There were no significant differences in the incidence of adverse reactions between the two groups.

CONCLUSION

Metformin combined with intensive-exercise diet therapy significantly improves the glucose and lipid metabolism and islet function of renal cancer patients with diabetes and effectively enhances the 12-month progression-free survival. Further trials are, however, required prior to clinical application.

目的

评估二甲双胍联合强化运动饮食疗法对腹腔镜肾部分切除术后局部肾细胞癌伴糖尿病患者糖脂代谢及胰岛功能的影响。

方法

选取 2018 年 1 月至 2020 年 12 月在我院肿瘤内科接受腹腔镜肾切除术的 120 例糖尿病合并肾细胞癌患者,按随机数字表法分为两组,每组 60 例,分别采用二甲双胍(对照组)和二甲双胍联合强化运动饮食疗法(研究组)进行治疗。观察两组患者的血糖、血脂代谢、胰岛功能、生活方式、临床疗效及不良反应。

结果

干预后,两组患者的空腹血糖(FBG)、餐后 2 小时血糖(PBG)、糖化血红蛋白(HbA1c)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)均明显降低,且研究组降低更显著;治疗后,两组患者的高密度脂蛋白胆固醇(HDL-C)、空腹血清胰岛素(FINS)、稳态模型评估的β细胞功能(HOMA-β)均明显升高,且研究组升高更显著(<0.05)。干预后,研究组的健康促进生活方式量表Ⅱ(HPLP-II)评分和 12 个月无进展生存率均高于对照组。两组不良反应发生率差异无统计学意义。

结论

二甲双胍联合强化运动饮食疗法可显著改善糖尿病合并肾细胞癌患者的糖脂代谢和胰岛功能,有效提高 12 个月无进展生存率。但仍需进一步的临床试验验证。
