OBJECTIVE
To analyze the nurse's performance in the hospital discharge process of children with chronic disease.
METHOD
Qualitative research conducted from May to August/2019 with ten nurses working in a pediatric unit of a public hospital. The data, collected through semi-structured interviews, were submitted to inductive thematic analysis.
RESULTS
In the preparation for hospital discharge, nurses recognize health education as their attribution, valuing family knowledge in the training regarding home care. Nevertheless, their practice is weakened by the little articulation among the multiprofessional team, reduced nursing sizing and disarticulation in the health care network.
CONCLUSION
There is a paradox between the report and the practice of the hospital discharge process accomplished by nurses. Although they have theoretical knowledge, their work process is influenced by the disarticulated way in which the multiprofessional team interacts, and the conditions provided by the institution to work in the preparation for hospital discharge of these children.

分析护士在慢性病儿童出院过程中的表现。
方法
2019 年 5 月至 8 月期间，对在一家公立医院儿科病房工作的 10 名护士进行了定性研究。通过半结构化访谈收集数据，并进行了归纳主题分析。
结果
在准备出院时，护士将健康教育视为自己的职责，重视家庭在居家护理方面的知识。然而，多专业团队之间的联系不紧密、护理人员配备不足以及医疗保健网络脱节，削弱了他们的实践能力。
结论
护士在完成出院过程中的报告和实践之间存在矛盾。尽管他们有理论知识，但他们的工作过程受到多专业团队互动方式脱节以及机构为这些儿童准备出院提供的条件的影响。
