This study aims to ascertain the nurse's role in the discharge process of children from hospitals, in the articulation between hospital care and primary care, from the point of view of comprehensive care. The methodology has a qualitative exploratory descriptive approach. It uses semi-structured interviews with seven nurses of hospital care and six of primary care, collected from December 2008 to January 2009, in their own workplaces. Results show a gap in the post-discharge nursing process, because there is virtually no articulation between hospital care and primary care. Primary health nurse often do not act in this process, because it is difficult to identify the child as discharged from a hospital. The actions of each nurse can not always go beyond their workplaces. Professionals involved in this process acknowledge their limitations where this articulation is concerned, and this documents the necessity of a reorganization of the work process.
本研究旨在从综合护理的角度,确定护士在儿童出院过程中以及在医院护理与初级护理衔接方面所起的作用。该研究采用定性探索性描述方法。研究于2008年12月至2009年1月在护士们各自的工作场所,对七名医院护理护士和六名初级护理护士进行了半结构化访谈。结果显示出院后护理过程存在脱节现象,因为医院护理和初级护理之间几乎没有衔接。初级保健护士往往未参与这一过程,因为很难确定儿童已从医院出院。每位护士的行动往往无法超越其工作场所范围。参与这一过程的专业人员认识到他们在这种衔接方面的局限性,这证明有必要对工作流程进行重组。
