Abstract

BACKGROUND

Hepatocellular carcinoma is one of the malignant tumors with the highest incidence in the world. According to the latest statistics of the National Cancer Center, the incidence of liver cancer ranks fifth in malignant tumors and its mortality rate ranks second in China, which seriously threatens people' s life and health.

AIM

To investigate the value of CT perfusion parameters and apparent diffusion coefficient (ADC) of magnetic resonance imaging (MRI) diffusion weighted imaging (DWI) in the diagnosis of hepatocellular carcinoma.

METHODS

43 patients with hepatocellular carcinoma and 40 patients with hepatic hemangioma treated in our hospital from August 2018 to August 2021 were selected for CT perfusion imaging and MRI examination.

RESULTS

The liver blood flow (BF), liver blood volume (BV), and hepatic artery perfusion (HAP) in the hepatocellular carcinoma group were (267.38 ± 35.59) ml/(min·100 g), (30.20 ± 8.82) ml/100 g, and (0.64 ± 0.10) ml/(min·ml), respectively, which were significantly higher than those in the hepatic hemangioma group ( < 0.05). The ADC value of hepatocellular carcinoma DWI sequence was (1.20 ± 0.17) ×10 mm, which was significantly lower than that of hepatic hemangioma ( < 0.05). The area under ROC curve of BF, BV, HAP, and ADC values for hepatocellular carcinoma was 0.860, 0.754, 0.804, and 0.890, respectively. The area under ROC curve of the four groups was compared ( > 0.05).

CONCLUSION

CT perfusion parameters BF, BV, HAP, and DWI sequence ADC values have certain application value in the diagnosis of hepatocellular carcinoma, and there is no significant difference between the diagnostic value of each parameter.

背景

肝细胞癌是世界上发病率最高的恶性肿瘤之一。根据国家癌症中心的最新统计,肝癌的发病率在恶性肿瘤中排名第五,死亡率在中国排名第二,严重威胁着人们的生命和健康。

目的

探讨 CT 灌注参数和磁共振成像(MRI)弥散加权成像(DWI)表观弥散系数(ADC)在肝细胞癌诊断中的价值。

方法

选取我院 2018 年 8 月至 2021 年 8 月收治的 43 例肝细胞癌患者和 40 例肝血管瘤患者进行 CT 灌注成像和 MRI 检查。

结果

肝细胞癌组的肝血流量(BF)、肝血容量(BV)和肝动脉灌注量(HAP)分别为(267.38±35.59)ml/(min·100g)、(30.20±8.82)ml/100g 和(0.64±0.10)ml/(min·ml),明显高于肝血管瘤组(<0.05)。肝细胞癌 DWI 序列的 ADC 值为(1.20±0.17)×10mm,明显低于肝血管瘤(<0.05)。BF、BV、HAP 和 ADC 值对肝细胞癌的 ROC 曲线下面积分别为 0.860、0.754、0.804 和 0.890。四组 ROC 曲线下面积比较(>0.05)。

结论

CT 灌注参数 BF、BV、HAP 和 DWI 序列 ADC 值对肝细胞癌的诊断具有一定的应用价值,各参数的诊断价值无显著差异。
