Abstract

OBJECTIVE

To determine the clinical value and feasibility of CT pulmonary angiography (CTPA) with personalized injection of contrast agent in pulmonary embolism (PE).

METHODS

In the present retrospective study, 130 patients who underwent CTPA examination in our hospital from June 2019 to May 2020 were evaluated. Among them, 67 cases were detected by CTPA with personalized injection of contrast agent as the observation group (Obs group), and 63 cases were detected by CTPA with bolus-tracking (BT) as the control group (Con group). The specificity, sensitivity and accuracy of the detection in the two groups were compared. The image quality score and superior vena cava artifact score of the two diagnostic methods were compared. Additionally, the volumetric CT dose index (CTDIvol) and dose length product (DLP) of the two groups were compared.

RESULTS

The Obs group yielded a significantly higher specificity in diagnosing PE than the Con group (P<0.05), but there were no significant differences between the two groups in the sensitivity and accuracy (P>0.05). The image quality score and superior vena cava artifact score of the two groups were not significantly different (P>0.05), and the Obs group showed significantly lower CTDIvol and DLP than the Con group (P<0.05).

CONCLUSION

CTPA with personalized injection of contrast agent has good diagnostic value for PE, with good imaging effect and safe profile, and has a lower radiation dose requirement.

摘要

目的

探讨个性化注射对比剂的CT肺动脉造影(CTPA)在肺栓塞(PE)诊断中的临床价值及可行性。

方法

本研究为回顾性研究,对2019年6月至2020年5月在我院接受CTPA检查的130例患者进行评估。其中,67例采用个性化注射对比剂的CTPA检查作为观察组(Obs组),63例采用团注追踪(BT)的CTPA检查作为对照组(Con组)。比较两组检测的特异性、敏感性和准确性。比较两种诊断方法的图像质量评分和上腔静脉伪影评分。此外,比较两组的容积CT剂量指数(CTDIvol)和剂量长度乘积(DLP)。

结果

Obs组诊断PE的特异性显著高于Con组(P<0.05),但两组在敏感性和准确性方面差异无统计学意义(P>0.05)。两组的图像质量评分和上腔静脉伪影评分差异无统计学意义(P>0.05),Obs组的CTDIvol和DLP显著低于Con组(P<0.05)。

结论

个性化注射对比剂的CTPA对PE具有良好的诊断价值,成像效果好,安全性高,辐射剂量要求较低。
